lost 40 lbs - should I adjust settings? charts included

Post by cake321 » Sun Mar 02, 2014 6:35 am

I lost about 40lbs. My previous settings were minimum 11, maximum 20. I changed the minimum to 8, and this was my chart for that night:

My max was 9.11, 95% was 8.92, AHI was .44

I then changed the minimum to 6, here is the chart:

Max was 8.54, 95% was 8.24, AHI was .58

So, I am pretty sure my old minimum of 11 is too high, but I am unsure of what to set it to now. Should I just set it to 8 since my 95% was between 8-9 both nights? I dont think I need to experiment with lower minimums than I've already tried.

Thanks in advance for any suggestions.

What about experimenting at levels decreasing by one # at a time for a few nights (at least) on each setting, and seeing which one is the most likely to be appropriate in future? You can always re-tweak when necessary of course.

Can't get much lower than 6 cm minimum. That's good.
I don't see the need to set minimum up around 95% pressure when you are so close to it at 6 cm minimum...now if the 95% was up around 12 or 13 long term (not just for a couple of nights) then we set it closer but you are so close now and the events are well controlled if you AHI is less than 1.0.
I would use this lowest minimum pressure for a while to see how it works out. Need several days worth of data.

Looks like the pressure increases when they do happen might be happening during REM stage sleep.

Congrats on the weight loss.

So my minimum should actually be a few CM below my 95%?

Yep, that's the way I would do it.
If a person keeps increasing minimum to equal 95% pressure they are chasing a moving target. Each time you raise the minimum the law of numbers will increase the 95% number.
Now if you were maybe going to use cpap mode...then yeah, pick a number close to the 95% number.
In auto mode...2 or 3 cm below 95% number will normally work just fine and your AHI of less than 1.0 sure affirms that.
Now if your AHI was maybe 6.0 something...then we might want to increase the minimum a bit but from what you are reporting with low AHI...more minimum isn't needed.
I always like to use the least amount of pressure as possible that gets the job done and let's me sleep well and feel decent.
Now if you didn't sleep well or feel decent then we have a different discussion but based solely on AHI..more minimum doesn't appear to be needed.
That 95% number isn't the holy grail of numbers like some people might think. It's just a number that sometimes if we were using cpap mode we would want to use.
In APAP mode it's just a number that is easily skewed so I don't put much stock in it unless I see long term 95% numbers...like at least a month.
