My Sleephead info from last night

I have been on xpap for almost 6 years now and have just gotten an S9 about a month ago. Changed masks at that time also because I was getting a lot of leaks from my Tap Pap and high AHI's between 12 and 20. Now using a quattro Air.

I have had some nights under AHI 5 but more then 1/2 are over. I'm getting a lot of clear airway events and they dominate the events each night.

I currently have torn rotators in both shoulders and sleep mainly on my back however the past week been trying to sleep more on the best of my shoulders, I have not had beer or any pain meds or sleeping meds in the past week getting ready for surgery. I'm an active person and just maybe 10-15 lbs heavier then my Dr. would like me.

Whats up with all of the clear events? What questions should I be asking the sleep Dr.? I will be seeing her in about 2 weeks

Almost would like to go back to the M series and live is Blistful ignorance.

Currently my settings are 10-15 with a fifteen minute ramp starting at 8. Flex is set at 3 and climate control at 80.

Are you for sure asleep when the centrals (clear airway events) show up or are they perhaps showing up at the beginning of the night while you are awake or just drifting off to sleep?

When I was using Quattro Air I had a lot of leaks and centrals. Now I'm using Nano nasal FX with a chin strap and my AHIs and Leaks are way way down. My AHI last night was 0.0.

Ok here is the theory behind high numbers of CPAP induced central events. Sorry to bore anyone with a lot of detail and those who have already read my previous posts on the subject.

As Pugsy says central sleep apneas can occur normally during the period between being asleep and awake as the body changes the CO2 apnea threshold level. When asleep (in the absence of cardiac problems) many central sleep events are caused by CPAP induced hyperventilation which lowers the CO2 levels close to or below the CO2 apnea threshold and reduces or stops breathing. This is similar to altitude induced central sleep apnea, which occurs in everyone depending on the altitude, and is caused by low O2 driven hyperventilation. As a result the hyperventilation and central sleep events increase with increasing CPAP pressure.

The best way to treat these central events is with an ASV machine. However there are a couple of things that can be tried with conventional CPAP/APAP machines and have been shown to work for some people. Using a mask with higher resistance to flow or for some, breathing through their nose rather than the mouth, can increase the flow resistance enough to reduce the hyperventilation and reduce the centrals. The other, more practical way, is to increase the expiratory flow resistance which decreases the volume of expiration thereby decreasing inspiration and reducing the hyperventilation for any given CPAP pressure. This is why some people will significantly reduce their central events by switching off the expiratory relief (CFLEX/AFLEX/EPR). This is easier to tolerate at lower pressures but can still be achieved at higher pressures by a reduction in the CFLEX/EPR levels.

Sorry to go on but this has been shown to be helpful for some.

IIWY I'd wait till surgery was over and you're fairly tuned up before I'd do any dial wingin'.

Just got an S9 last week and was wondering about my incidence of centrals. I was not expecting to have many (any?) central apneas, but I was never able to read the data from my old S8, so I cannot use that old thing as a basis for comparison.

Hoping it's minor or transient, nothing major. Considering the raw number and the AHI (nothing over 3 in the 4 nights I've used it) I suspect it's nothing to be concerned about.
