Tired of being tired

I have been using my cpap since the end of August for 5-7 hours a night. My AHI is around 1.7 but I still wake up feeling tired. I know that there is sleep debt but I would have expected that not to be an issue anymore. My primary ordered complete bloodwork (fatigue profile) and all was normal except slightly low vitamn D levels. I don't have a morning headache anymore and my blood pressure is comming down but I am still tired. Do you have any suggestions?

What was your vitamin D level?

Don't remember but have been on RX supplement for 5 weeks. I know that it was not super low

I'm in same boat as you. I'm going to try to get mine up to 60. MIne was 15. The other thing to check on is B-12 deficiency

If you're using your Autoset in a range of pressures, I'd recommend switching to straight pressure (CPAP mode) and see if that helps.

I am using an autoset 13-17. Should I look at the print out and go for the average pressure and monitor the AHI's

Why would switching to a fixed pressure make a difference?

I'd try using the pressure that's somewhere between the "average" and 95%. And, yes, I'd also look at the printouts for some comparisons.

Some people can handle the changing pressures of an APAP and some/many can't. Our breathing changes as we enter into deep sleep stages and REM. This may trigger pressure increases which can bump the user out of those much-needed sleep stages.........and leave them feeling tired and unrested. It's also much easier to become accustomed to this therapy when using a constant pressure. Changing pressures seem to add another dimension of problems for those starting out.

Don't get me wrong. There's nothing "wrong" with having an APAP, but the user needs to experiment to find out what works best for them. Your symptoms are very typical of someone using a pressure range that disturbs their sleep.

here are my 60 day average numbers

pressure median 12.5 95% 13.9 maximum 15.1

leak median 1.2 95% 24 maximum 15.1

ahi 3.2

single day read out is
pressure median 11 95% 13 maximum 18

Since starting cpap, I remember bits and peices of lots of dreams for awhile. Could this be caused by the change in pressure waking me out of rem sleep? I am suprised that the average differences are not that great.

Would 13 be a good pressure to try?

Another thing comes to mind is Thyroid levels. Did they check on those?

Yes they checked all of that as part of the "fatigue" blood workup. Thyroid, hormones etc were normal

I think 11, 12 or 13 would get you started. Just pick one and try it from at least 3 to 5 nights (or a week) to see what happens with the numbers. If you start at 11, you can always tweak it up a centimeter or so after you see what the results are.

Please keep us posted as to what happens and how you feel.

Is it typical for board certified sleep doctors to be of so little use? I was amazed that I was blown off when I told her that I was still tired. Who cares if my AHI's are good if I am still tired. It was an ent that wanted me to talk to my primary about having the bloodwork!!!!!

It seems that the only use I have for the sleep doctor is to have insurance continue to pay for the masks etc. They did buy the machine for me two weeks ago
