AHI varies a lot night to night ...

Post by Flyboy » Fri Mar 31, 2006 9:46 am

I am on a RemStar Pro 2 w/CFlex and FF Flexifit HC431 mask and heated humidifier. After three weeks at the prescribed pressure of 10 (CFLEX 3) I have observed my AHI get better and better (to nearly zero) for the first two weeks but then take a dive this last week (and I have been feeling more tired again). I still wake a dozen times a night. last night it was 8.0 (mostly OSAs)--while this is technically not a huge number, I sure feel the difference! Along with the higher AHIs, the number of snores recorded also climbed.

Should I try cranking the machine up a notch for a night or two?

If you're not getting much in the way of excessive leaks, bump it up 1 cm and check your data, snores should go down also AHI, just make sure you don't have more centrals.

I agree w/th, I would look at your leak rate report first. Also any dry mouth or tongue during the night or morning can indicate presence of mouth breathing, this can also would show up on your reports as a leak or large leak.

Compare your AHI reports to the leak reports. You might also try enabling the mask off alarm (off by default, second to last parameter in programming the machine). Enabling this would trigger the audible alarm (beeps) during the night to allow you quick diagnosis of the problem. If the alarm doesn't go off, then you can eliminate that as being the problem. Next thing I would try would be dropping the Cflex down to 2.

I also initially thought of leaks (thanks to all the great posts on this board) and they are still low and in line with previous weeks. I have mask alarm on but have great compliance--I never wake up with the mask off. Since I have a full face mask (I tend to blow out the air so chin strap does nothing for me) dry mouth is not an issue.

Last night I bumped up to 11 and cut events in half. Feeling sharper this am. Snores are still huge...

I'll try setting CFlex lower--I tried this before and it had no effect on my numbers at the time.
