Please comment on my graphs, Ca's Re's, Hyp's, no Ap's

Post by aben » Sat May 26, 2012 1:18 pm

I have been using straight CPAP for 5 years and started PR S1 Auto Bipap Biflex 750 a few weeks ago. My pressures are 6, 8, 10. I set the max at 10 because higher causes aerophagia. I usually wake up a couple of times and turn the machine off and walk off aerophagia. I am using a Comfort Gel mask right now. It looks to me like the Ca events start then my flow rate goes down and I wake up. I am interested in opinions on my numbers particularly the Ca's, Re's, Hypop's, and no Ap's. I have not learned how to integrate my oximeter (CMS50D+) into SH yet.
Thanks.http://www.flickr.com/photos/78023783@N ... 274614426/

Sorry, I see that I goofed on the attachments, the two images that I wanted to send are there but they are duplicates. I will try to get it right next time.

Are these typical charts of your leak rates? That is something to work on - I don't know anything about bipaps, so whether they are normal or not I don't know. Leaks seem to be a critical success factor for me in getting the right therapy (straight cpap) - maybe something to look at?

Your other report's leak line was better but this last report wasn't horrible. I doubt it impacted therapy much.

Actually all your numbers are acceptable. That one cluster of CAs...that is unusual for you and not something we have seen on your other report with any great frequency. Since it happened prior to waking up...I am wonder if you were semi awake during that cluster of clear airway events. At this point, since it is not a common theme in your reports, I would disregard it.

Your other numbers...the RERAs, FL, hyponeas and apneas...well within acceptable limits.
While some people might want to "clean up" things a wee bit more, it really isn't need and do try to clean it up (reduce a few more) you would need more pressure (EPAP) and it would create more aerophagia.

Since your numbers are well within acceptable limits anyway and there is no pressing need for any changes then other than working on the leaks (always have to do that). If you look at the leak line...the increased leaks occurred right before the CA cluster...I suspect that the leaks caused some level of arousal which lead to waking up. Can't prove it...just I suspect that even though the leak isn't high enough to mess with therapy, it likely is/was large enough to disturb your sleep at some level.

Last night's report..again I don't see anything of great excitement. That last little cluster right before turning machine off I suspect you were either awake or waking up. You don't really need the AHI graph..you can turn it off in preferences so that you can see your leak line at a glance. You can already see the events in the top event graph.

Are you having issues other than aerophagia? Your reports are quite acceptable.

Thanks again pugsy,
Occasionally I wake up with tachycardia and nausea but it is getting better. I showed my sleep doc some SH graphs and asked him if I could send him reports once in a while. He didn't seem to know much about them and said that I would have to send them to my DME. I wish that he would be willing to do it.

Your DME or doctor are not going to be familiar with SleepyHead and the way it presents the data.
They should be familiar with Encore software report.
Mostly what they will look at is hours of use and maybe the AHI and the event category breakdown. They don't put these reports under the microscope that we do.

What about the pressures graphs that have sawteeth shape?

That is normal appearance for the Respironics APAP pressure line.
Minor increases in pressure to test things...if it doesn't sense anything exciting it comes back down.. This probing is done regularly if it finds a reason to stay up then it stays up or goes farther. Sometimes all that is seen is the sawtooth pattern.
