New PR System One question

Post by detcord » Sat Jan 07, 2012 8:44 pm

About a month ago My first gen system one gave up the ghost, and was replaced with a new one. I immediately noticed that the flow seemed "gentler" Since I run a flow between 16.5 to 2o, with a median rate of 18, this was very welcome. My AHI before generally ran around 5, and now with no changes at all, are now around 3. Just wondering if anyone else has noticed any changes going from an older PR system one, or is it just me. As to the AHI change, has the way it counts AHI changed?

Hi detcord

Yes, it's a difference on feeling. As you noticed the newer generation pressure delivery is gentler as it's the pressure pulses. Me too I had one of the first generation and changed for a newer and noticed the difference. But my AHI had no variation. +

What was the problem with the older machine? I had one (one year old or so) which had problems to deliver humidity and it was not the humidifier's fault, I hope the new will last a good 5 years . I get replacement and it is working properly.

Most of us haven't had a chance to compare (but a few have) a new PR S1 machine to an older model of the same machine.
Repironics did come out with a revision this past spring. The internal firmware was changed and they added a LCD backlight,& a bit more insulation to help with noise. I don't thing they made any changes to the algorithm. It is possible that with these model version changes that you might perceive a little difference. Maybe all of this adds up to resting better and the machine doing a better job.

So there were some changes made likely from a machine given in early 2010 and one given recently. Enough to notice a change beyond what they say they changed as to change AHI....I don't know.

I had the first gen and now have a later one. First Gen did the stop start thing in the middle of the night after about a year so they swapped it for a new one. Interestingly I'm worried the next gen has just started to do this too recently.

I did notice changes, but when I got my replacement I also decided to change a few other things so it may well be a combination of things.

1. Average pressure is now around 9.5 but back using the first gen machine was around 16. Machine may be partly responsible but I suspect weight loss has had the greater impact.
2. AHI now under 1, was around 3.
3. Was gradually able to move my starting pressure up from 6 to 7.5. Had tried this previously and had struggled with the pressure at the start of the night. The change may be because of the "nicer feel" you describe or it could be I was just more tolerant after a year of use.

I'm on my second System One, but suspect that both are what you refer to as "first generation". I'm not sure what defines "second generation".

As for your changes in AHI, could it be that one (or even both) of your machines were delivering inaccurate pressures?

Another thought is that maybe the two machines score apneas differently. IOW, the therapy is the same but a different yardstick might be used to measure it.
I'm workin' on it.

I suppose one could think of "generations" in regards to the PR S1 machines the first models with lack of LCD back light and older firmware and the newer models with different firmware and the LCD backlight availability.

I've had mine replaced twice because of problems with erratic algorithm "pulsing".The machines were tested and found to be faulty. FWIW, all of mine had the backlight.
The third one they sent me is still sitting in the box, unopened. I don't intend on using it unless my old Remstar Auto dies.I've never had any problems with that machine.
