Help with numbers please...

Post by tmorris » Sat Nov 26, 2011 6:49 pm

Hi everyone..I am not good at all with these numbers. I need your expert advice. I am still using the Resmed S9 auto cpap as shown on my signature. I am also using the swift LT nasal pillows...leaks are okay i think. Lately ranging from 2.4-4.8. I was titrated at a straight pressure of 12 which the doc said needed to control my apneas. However, I am unable to tolerate that and changed it to AUTO myself. I go to a different doc next week (long story) but I would like your opinions on here. I am now having trouble with the horrible gas and bloating once again. It was alleviated when I put bricks under my bed legs to incline the bed...but it damaged the bed so my husband took them out. so now I use a wedge pillow with not much success...I have been sleeping in the recliner some lately since this is the only solution to keep the gas pains away. And I am referring the so much air in my stomach, back, and chest that I can barely walk! Anyway, I have my machine in auto mode set at a range of 12-14. I am hoping I can put the min pressure down a bit with good results. I just need some recommendations on what numbers to try...I did not do well with a broad range as I felt horrible the next day. My statistics read the following (mainly the same) for the past week: Pressure: Median-12.4, 95th percentile-13.5, and max 13.9. AHI-1.0 (sometimes 1.5) I am still getting very tired mainly the late portion of the day which I will discuss with my doc. I use Nuvigil as needed. I would like recommendations on a range of pressures to use for auto that would alleviate the air spilling into my stomach (which I plan to get checked out as well). I guess Im concerned with lowering the min too much ...It looks like I hit my max pressure nightly and my tummy hurts! Thanks for any suggestions. My EPR is full time at 3

So you were tolerating 12-14 with the bed legs elevated, but not with the wedge? Is the angle of the wedge the same as the bed, or was the bed higher? Some of the wedges that are short don't do well for people who have GERD because they make you bend at the middle (so say the manufacturers that want you to buy their longer, more expensive wedges). I am not sure what to tell you because 12-14 is exactly what I'm supposed to be using and I can't even approach those numbers without aerophagia myself... You can try various things:

(1) lower, constant pressure (e.g., 11 or 10)
(2) lower minimum pressure and keep the maximum the same (but you seem to feel you feel tired like that...)
(3) lower minimum pressure and lower maximum pressure

Do you have GERD? If so, you could throw a proton pump inhibitor at this and hope it might help.

Wish I knew the answer, for both of our sakes...

What do you mean by not being able to tolerate your titrated pressure of 12 on cpap?
If you couldn't tolerate 12 on cpap, why did you increase the pressure to 14?
Unless you were having a very high AHI on the cpap at 12, I would try the the auto at 9-12 or 10-12, and see if you get some relief from the aerophagia and what your numbers do. Are you checking your data in ResScan?
