Don't be shy!

Who has the CPAP-Talk record for the Highest AHI, Lowest Desat, Longest Apnea?

What about the highest pressure setting?

My AHI is 20 with minimal desats and 10-15 second Apneas, pressure Auto 13-20cmH2O. I know, weak.

Bet you can do better. Do you have the record?

124, 26% and 119 seconds, respectively (although not me personally. Clearly, one could not seriously compete in the Longest Apnea and Highest AHI categories simultaneously).

I hadn't thought about it but having super long Apneas would actually lower the number of events that can fit in an hour. Thats one way to lower your AHI...just have longer apneas!

Ha! I have COPD, require 2L of 02 supplementation w/my bi-level - and none of my titrations have ever come close to you guys for number, length or desats. I'm more than happy to NOT be a candidate for close competition in this poll!!!

I must be in the middling category:
62, 83%, no apneas during PSG & 22 secs on my AutoSet II sleeping on my back (shoulder injury), auto range 9.8-16 (I have maxed out at 16 when on back), 95centile is 10.5 for the past yr and I think around 12 since I fell last July).

That's a long, long time. Takes some steel nerves and a LOT of knowledge/experience to sit and watch a sleeper like that.

Mostly patience. Taking all comers, "IMHO" the odds of seeing guys like that are about 6000 to 1.

However, the search becomes significantly easier by profiling-- obstructive component, CompSAS tendency plus circulatory delay will usually yield some fair-sized apneas.

ahi 30 LA 62 secs LD 94% ... only possible with a pulse rate in the 30's !

Agreed... But now we have to find out if Bev even WANTS to go to Dairy Queen... It's entirely possible she prefers a fine wine or Dunkin Donuts as her long-apnea reward.

That said, this sounds like a job for the TSPUGRC---our well-respected That-Said Phrase Users Group Reward Committee, of course.

That was a great call IMO...

BTW, if central-apnea length is a facet of severity, I think Bev's central SDB component can be characterized as "infrequently severe." And despite that infrequency, I think her ASV provides her with a very necessary backup rate. Look at what happens to her respiratory drive beginning right around t=2:30 in the thread below. See BPM and trigger rate as respiratory-drive markers; note that tidal volume artificially spikes since IPAP peak maxes out:

In retrospect, Bev seemed to largely acclimate to both CPAP and APAP. But she never managed to avoid those infrequent but dangerously long central apneas until finally acquiring an ASV machine with backup rate.

My stats don't compete either, but here they are

11 AHI in non REM sleep but 52 AHI in REM sleep
longest event 45 seconds
desat to 73%

I'm not sure of the exact values, but AHI was mid-90s (almost 20 years ago). Longest apnea then was not too long. Unfortunately, as the central apneas have grown, my central apneas tend to last a long time. More than a minute. My M Series BiPAP would cut off after a minute. The fact the Auto Off feature kicked in while I was wearing it was a sign something was wrong. (No breathing must mean no one is attached!) It would cut off several times per night. Lowest desat prior to ASV was around 68%. But I know the apneas were frequent enough to cause me *serious* headaches most days. I felt horrible.

My ASV unit has made all the difference in the world. No headaches. Controlled blood pressure. I definitely feel better.

Now when I fall asleep, I usually note (with detached interest) that the unit starts ramping up the pressure, since I always have significant central apneas as I fall asleep. It's odd how I've grown to *completely* trust the unit will help me breathe. Though I note the change in pressure, most of the time I just continue to drift onto sleep.
