NEW development, having a hard time updated 8/31

Post by KC5cychris » Mon Aug 30, 2010 2:54 pm

uAs of late, I have been having a very hard time limiting the amount of leaks that I have had. I have tried tightening the straps loosening the strap, the anti leak strap, mask liner. I ever went and got a new cushion and tried again this morning it did not seem to help I am running in APAP mode 12- 16cmh20 as the pressure comes up to 12 I am ok its through the night when the pressure raises to chase to rem induced events that the leaks seem to go crazy, then of course you wake up and try to stem the leak by resetting the mask or tightening a strap or 2 any ideas would be appreciated

Chris

Have you tried either a different size of your mask or a different one entirely lately? Have you lost weight (it can make a surprising difference in your face), or not using wipes (if nothing else) on the mask cushion?

You might try temporarily upping your minimum pressure to 15 or 16 and then using the Mask Fit function to adjust the mask at the higher pressure. (If the S9 is like the S8, holding down the start knob for a few seconds will start Mask Fit. If i'm wrong, someone will chime in and correct me.) The display will show you how good of a fit you have.

Do this when you are wide awake, not at bedtime.

If you've checked all the mask adjustments, hose management, head pillow dislodging masks, then I would consider changing to CPAP Mode.

I can't tell you what pressure would be best. If you were titrated at 10, was that during REM and supine? If so, that would be a place to start, and you could adjust according to what your data says.

If you are using EPR, you would need to compensate for that, eg. 10 + 2 for EPR.

And if you are using Ramp, make it short, like 5 minutes, so you can adjust your mask as it goes up to full pressure. Or, use the Mask Fitting feature.

I was just going to offer the same suggestion but Katie really explained it better than I ever could for that machine.

Last night I ran in CPAP mode at 13 with a zero leak line my AHI was 5.4 with 2 large event groups at 2 and 4 am, I need to work on getting these under control, as I feel the best when my AHI is under 2. I was going to post some graphs but could remember how to covert pdf's to gif
anyway
any sugestions are appreiated
Chris

My only suggestion would be to stay with this setting for a few days and see what the trend is and how you feel. You know the old saying around here, "one night does not a trend make".

Zero leak line is great! I agree with Dori, stay at this setting for a few days and track it.

Is your AHI lower than it was before? What is the breakdown of CAI, OAI and HI?

I am at work so I will check when I get home and post. But I think most of it was obstructive

This probably sounds odd; but have you been starting your nights later than usual?
I know if I stay up too late, then I seem too tired to get comfortable with my equipment.
Good luck, Chris; maybe someone has an idea. Please trust it will get better. Bless you.
