Would you look at my titration report please?

Hi friends,

I had a successful PSG/tiration study last month. I dozed off at 9:00pm and by 10:00pm the tech came in and said “I’m here with your mask”. Apparently during the hour that I slept, I had over 100 apnea events, … holy smokes. She put a nasal mask on me and it took me all of 5 minutes to get used to it and I slept like a baby until early morning when she woke me up.

Got the report for both studies and I’m trying to digest it all. Would you look at my titration summary here and help me with a few things I don’t understand? Specifically,

1) Looks like she started me off at 5cm and increasing in increment of 0.5cm. My AHI started out at 105.2 and then fluctuated a lot between 5cm and 7cm. At 7.5cm and 8cm, my AHI was 0 but she only stayed here a about a minute each, was it because my SpO2 was in the 93’s?

2) She let me sleep for 4 hours at 9cm and I achieved 0.7 AHI and SpO2 of 96.7. Did she bump me up to 9.5cm and 10cm to see if my SpO2 would improve?

3) She bumped me to 11cm and let me sleep for another hour and I got 0 AHI and SpO2 of 97.4. This ended up being my prescription. Wouldn’t 10cm be just as good?

The tech didn't stay at 10cm very long...the most significant point is there is no REM activity at 10cm, but there is a 11cm. At 9cm you had just one event, but that is enough to bump you up. I've never seen 0.5cm bump on a titration report...1cm adjustments are the norm. Not sure why the tech moved you up so quickly (5 mins is more typical I think). They like to get you up to 10cm quickly, that's the sweet spot for most people. Yours is a textbook example.

The difference between 10 and 11 is noise. Think of the prescription more as the center point in a range of pressures plus or minus 2cm let's say. Remember, this is only a snapshot of reality...your actual mileage may vary.

While I don't know about the specifics of titration, they probably look at your EEG / sleep architecture as well as SP02... though in 1 minute I don't know exactly what they can evaluate.

But anyway, this is the perfect reason to get a data capable machine, whether CPAP or the APAP variety!

You may very well sleep differently at home, from night to night, and even from hour to hour.
