Hello,

I have had my CPAP for about 5 days now, I am just in the first 4 week trial period where I am running 10cm constant pressure collecting data before I go back to my doctor again.

My initial study returned 36 events an hour normal and 63 events per hour REM (both an average value) so I was classified as having severe OSI.

Since my first sleep test I have lost 10kg (22lbs) and still have another 8 or so to go to end up at about 90kg (198lbs) - I am 6' 3" or so. I don't smoke and have virtually given up alcohol - I eat fish and vegetables but no meat, water is what I drink 95% of the day apart from a couple of green/black teas.

My second sleep test pointed to the 10cm value as realistic and as far as I am aware I had very few events the whole night.

So I bought a S8 AutoSet II with a Quatro full face mask (because I am a mouth breather) and the following are last nights values;

I have a reasonably large 'dose' of insomnia and so getting to bed early is very hard (I think it's time to change that) but even so I still feel dramatically better after 5 days - but from what I read here my AHI is still high - on previous days it was as high as 12.

Any thoughts/ideas on how it is going would be appreciated.

One thing, I don't want to change any settings before I see my doctor again..... I don't think that he would be happy with me if I did.

Welcome to the board, Tony!

EPR was not used during your titration night at the sleep lab when they found that a pressure of 10 was what worked well for you. EPR "3" means you're getting a pressure of only 7 during your exhalations and (most importantly) during the pause before you start to inhale again.

What I'm writing next would not be changing your doctor's prescribed pressure setting...it would be changing a "comfort" setting -- the EPR adjustment:

You might want to try your machine in some of these ways:

EPR 1 (instead of 3)
or
EPR during Ramp only (instead of Fulltime)
or
EPR (off)

Tony,

Welcome back to the world of the awake people.

That doesn't look too bad for someone who has just gotten started. People tell me that Resmed machines score hypos (HI) a little high. In fact, some state that you can compute AHI as A + HI/2 for these machines. Using that I see 1.2 + 8.1/2, which is an adjusted AHI of 5.2. 5 is typically the cut-off number that folks quote as effective treatment. However, for most hosers, the A number is far more important than the HI number. Your A of 1.2 is very good. The suggestion of reducing your EPR from 3 to 1 might cause some of those HIs to be suppressed. At any rate, I think things are looking good for you, at least for this test. The data that is collected will allow your doctor to suggest some tweaks. There is often an adjustment period when starting on the machine, so these levels might need some tweaking down the line anyway.
