Do NOT uninstall 1.8 yet. You'll need it for the migration to work.
This install setup is based on my experience with a one-patient install of EP1.8, so if you have lots of patients the actual migration could take a very long while.
Double-click and install the Windows Installer 4.5.exe. It's a hotfix to insure compatibility:

Files extract.

The next few images show what you'll see during the Hotfix install.

Once the Hotfix has been installed, Install the EP2setup.exe.

Obvious:

Another install-shield wizard:

Don't worry - you have to reboot to continue.

Of course once you've restarted, it resumes...

Continue installing...

Accept the EULA...

Make sure to click Advanced.

Just allow the default destination folder.

Make sure to check Data Migration.

Allow the default server name.

Forms Authentication is probably the only thing you'll have available, unless you're installing in an office with a domain.

Accept the EULA.

The obvious churning of the setup dialog box... This sits in the background for most of the install.

This little dialong will sit infront of the setup dialog box while it installs the SQL server. It takes a long time so don't get frustrated.

Depending on your firewall settings, you'll need to allow something twice. Hence the two next pictures. You probably should allow these permanently.

More dialogs about extracting the database.

Two more fire wall allowances.

A bunch more dialog boxes telling you what's happening.

Check both checkboxes.

The readme has your first user name and password. Use these to log in.

Once you've logged in, you'll need to immediately change your password. I forgot to make a picture of this, but it should be pretty self-explanatory. You won't be able to do the migration until you've logged in once and changed your password.
From the Start Menu, pull up to Programs --> Respironics --> EncorePro 2 --> Data Migration.
Log in. I forgot a picture for this too, but it should be self explanatory again.
Unless you have an unusual setup, choose express mapping, the default company and the default clinician. Click 'Next'.

Click 'Begin Import'.

This little dialog box comes up. It takes a short while and morphs into the next dialog box below.

This one just sits there when it's done. No real explanation that it's done. No beep. It doesn't even hand off to the originating window. The only way you know it's done is because the window for beginning import will magically say 'Mapping Complete' with 'Finish' as the only option.

The importer status dialog box will remain above the window. Just click 'Finish' anyway.

It won't be immediately apparent, but now you have to close the migration service. Right click it in the systray and pull up to 'Stop Service'. It might give you an alarm about stopping mid migration, but if there are no errors in the importer status, you should be good to go.

You are now ready to exit the migration service. Right click it in the systray and pull up to 'Close Application'.

Er. Um... Yeah. Click 'Yes'.

Go back to your folder with the install files. Double click 'EncoreSmartCardSetup.exe'.

Choose 'Transylvanian'.

It's a pretty short wait through this dialog box.

Install Shield Wizard for the Card Reader. Click 'Next'.

Accept the EULA. Click 'Next'.

Click 'Install'.

Processing...

A similar dialog box may pop up. Click 'Continue Anyway'.

Don't actually attach a device until you're finished with the install. This is just a reminder.

Click 'Finish'.

Now, insert the Mako Infineer USB Smart Card Reader into the USB slot. You'll see the following two notifications in your systray:

Do yourself a favor and reboot. That seems to work best.
Try out your new card reader with the myCPAP.com Card Utility 1.06. It works great as a backup utility.

Here it is completed. Now you know your card reader works.

Here's an image of the login as it looks on your screen. You're going to see those two people for along while. Images of people generally date software pretty quickly. In a few years you'll look at them and say, 'They're so twenty-aughts.' Why couldn't they just have used a pattern or a logo?

Go to the patient tab and select the patient from a list of patients. Sorry - I forgot an image of this. But here's the individual patient page. Pull down from the SD card icon to 'Download from Device'.

Select 'Infineer'. If you're migrating from 1.8, of course you'll be using the Smart Card Reader. Be sure to check 'Card is to be returned to the same patient,' and 'Leave data on card after download', unless you choose otherwise.

You might see something saying: 'This is not the same patient as the one on the card. Import to this patient anyway?' Allow this to happen. It will only happen the first time, because then the card will be assigned to this patient.
An example of the download in progress. Seeing a percentage seems to make it go faster, but it's not really any faster than 1.8.

An example of the download dialog box when complete. This will disappear on its own.

An image of the patient specifics expanded above the patient tabs.

That's all there is for the install. What's interesting with this version is that the main window is now un-resizeable, but the report-preview window is resizeable. There are some weird window implementations - definitely non-standard for windows, mac or linux interfaces.
The software is eye-stabbingly slow when you change to anything longer than a week in the therapy data tab. If it weren't necessary to upgrade to 2.2 in order to view data from System One, it would be a tossup as to whether this software is better than 1.8.
My card got messed up prior to installing 2.2, so a few weeks of data are recently missing. I had to reformat the card. The first time I installed 2.2, I didn't select the migration tools and uninstalled 1.8 first. That was a mistake. Fortunately, I make a backup image of my hard drive in case I needed to start over. I just restored from the image and did it right the second time. It was a lot of pictures. I hope this helps someone.
