Hose hanging down below my chin. I get less leaks. Over my head seems wierd to me and I got leaks that way. I use the hose cover made by Pad A Cheeks so it is nice and snuggly --under the covers with me and works great!

It may depend on the mask. With my Quattro if I have the hose over my nose it occasionally blocks the vents and makes noise. Since I sleep on my side it works well for me to have it hang down the side of the bed. When I wear my Nasal mask with the PapCap, the hose comes over my nose.

IF the hose with cover didn't partially obstruct the air flow (which it does) I'd go for over the head right now. Since it does, I don't.

It's going to be okay in the end; if it's not okay, it's not the end.

Hose below chin for me. When I route it over my head, it seems to apply pressure on my Swift LT pillows and I get leaks. Almost like a lever. With it down, that doesn't happen. I am also using The Hozer hose management system.

I agree that different masks seem to do better one way or the other, so you probably need to try both and decide which works best for you. My experience was that the one night I tried the hose up, I had massive rainout. Ditto those above that I had more leaking that night as well. With the hose down and under the covers, I have never had a drop of rainout.

I use my ComfortGel either way, although I prefer to have it swiveled upward.

Below the chin for me too. My c-pap machine sits on the floor, so this works best for me. I also put the hose under the covers, Hardly ever have any rain out problems.

You didn't say which mask.

But, whichever way works better for you.

My hose elbow is pointed downward because if the hose goes upward over my head, the exhaust will hit the hose and make undesired noise.

I put my over my head. For me it's more comfortable and out of the way.

I take option 3.

When I begin sleep on my back, the hose starts out below the chin and veering left towards the table and the BiPAP machine, in a U hanging over the floor; but when I turn onto my right side it often ends up swiveling to point out over my left cheek, hose now trailing "behind" my head. Despite all the other acclimation problems I still have, the hose -- which like my head remains outside the blankets -- never seems to bother me wherever it chooses to lie.

Part of the "geography" is that I sleep on the left 1/3 of the bed (as seen from in the bed), adjacent to the table where the BiPAP resides.

Over the head routing works great for me.

As others have said, sometimes what works best for a particular mask dictates which way will produce more or less noise, and more or fewer leaks. Or rainout.

Experiment until you find what works for you.

So many people, so many variables!! I was going along fine until I hit hunting camp and it was 30-40 degree air in the tent, being sucked into the machine. That when you switch to hose under the covers to warm the air going in your nose!! The flip side of that was when I changed tent heaters and took a WAY TOO BIG woodstove in there. Yes, it was nice a toasty at bed time, but it quckly became apparent that my CPAP was too close to the stove and was burning my nose!! The metal ammo box my battery and machine live in was too hot to touch!! YIKES!! The nice part I like about winter camping and CPAP is that I can burrow under the covers and not worry about getting outside air that's "just right", since you're gonna get the same air pumped to you no matter where your head is, under the covers or out!

It's all preference. On guy's answer is the next's nemesis!!

I use the ComfortGel Nasl mask. The mask has a 360 degree swivel where the hose connects to the mask. This allows the hose to freely move when I move my head from the left to the right. I'm a stomach sleeper so this swivel feature make hose management a breeze.. When my head is facing to the right, the hose is down over my chin. When my head is facing right, the hose is directed over my head. I can hear the mask exhaust when the hose is over my head. At first, I thought the exhaust was a leak around my eyes. I confirmed the exhaust when I ran the data throught the Encore softeware. My mask is performing within normal leak limits.

I route the hose above my head. I modified a coat hanger to hold it on the bed's headboard. That way the hose is always free and never tugs at my mask when I turn to either sides. I sleep on my back 60-75% of the time but often sleep on my sides too. When I was titrated at home I had a full face mask and the hose went down and it was always stuck in my covers even when I was just turning my head. I hated it. Now I can move as much as I want to and never worry about my mask moving and leaking.
