I use the Hozer, so the hose hangs on a thin flexible rod (like a fishing pole). I adjusted the hose so there is no slack hose lying on the pillow, but the pole still gives me room for movement like changing sides or sitting up.

I too, use the Hozer system and really like it. With it, my leaks went down to consistently almost nothing, and my AI's followed suit. It was an excellent investment in me that I highly recommend

I also use the Hozer, so that eliminates the problem for me. But if you're looking for a smaller pillow, you may want to take a look at the Bucky pillows: http://www.bucky.com/catalog/bed_pillow ... buckwheat/. They have several types. There's one that's all buckwheat hulls, and another (in two sizes) that's a duo pillow--double-sided, filled with 50% buckwheat hulls and 50% millet hulls. You can remove hulls to suit your preferences. Standard pillow size is usually 20" x 26". The Bucky pillows are 15" x 20" and 11" x 14". A standard-sized pillowcase wouldn't fit snuggly with those, but that may not matter much. Some of the pillows at cpap.com look smaller than standard too, but I don't know for sure: https://www.cpap.com/simple-find-cpap-p ... wNumber=30.

You could try cutting down a standard pillow cover (by half or whatever), sew the new seam, and fill with whatever material is most comfortable for you, such as buckwheat or millet hulls, latex foam (my favorite, as it's soft yet resilient and it breathes), memory foam, polyester fill, down, or whatever. Carbonman has some photos posted that show his slashed memory foam pillow design, but he said it seemed to lose its "memory" over time.
Let us know what works for you, secret agent girl. I'd be interested in knowing if you experiment and make one yourself. I think most of us are searching for that "perfect pillow" at one time or another!

I would research the size and firmness of any cpap specific pillow before buying one. Many of them are very firm and so high that they seem to be to made for guys who are over six feet tall pillow because of their neck and shoulder size or for those who have a personal preference for such a size/firmness. Get the dimensions and firmness then trying raising a standard pillow to see if your neck/shoulders can tolerate a high firm pillow which I found uncomfortable and seemed to put airway restriction on my short heavy neck. Was able to check out one on display at a local DME and glad that I did not waste money ordering one.
I found pillows made of down and some of the synthenics are very supportive and conforming. The pillows made of buckwheat and/or beads are okay but can make it crunchy noise plus are not machine wash/dryable.

100% silk pillows, the only way to go baby! Warm, quiet, pliable (not as manipulable as a buckwheat, but more than sufficient for me), soft, supportive, and not 4" or more tall like all the "cpap speciality" pillows I ordered when I first got my cpap. Even my oversized one is not that tall, and the original versions are downright flat. Some people buy the regulars and fold them in half. They are heaven. I am sooooo glad that I did not have to give up my silk pillow when I went CPAP. I am still in pillow heaven!
FWIW, my mask lies on my pillow or in the little indentions that I make in my pillow, and I like it that way. I cannot sleep on the edge of a pillow or stand to have the weight of a mask hanging off of my face.

About six or seven years ago I picked up a few of the silk pillows and the brand was: Cocoon. I still have several of these, they are great but at that time they only had the very flat type. A few years ago I wanted to see if I could find one that was a little thicker. My husband puts two flat ones inside one pillowcase and loves it, but that was too thick for me, I needed a happy medium.
So I couldn't find the Cocoon brand anywhere. I came across Kumi Kookoon, and as luck would have it they have the regular silk pillows AND a supersize version which was the perfect thickness for me. Here is a link to their website: http://www.kumikookoon.com/
They are carried in stores, particularly the speciality bedding stores, so you might be able to track one down in person to check it out before ordering or purchasing. Be careful: they do not allow returns if ordered directly from their website. Drat! But I can vouch for their very high quality.
Now I am not Big Bank Hank, but as someone who was spending inordinate amounts of time in bed as an undiagnosed OSA victim...I eventually took to spending $$$ so that I could *finally* get some decent sleep. My house is like a bedding museum. I have organic cotton, organic wool, organic rubber pillows & comforters. And down! And foam! Featherbeds, mattress toppers, mattresses. UGH if only I'd known all I needed was a CPAP.
But the silk are by far the best imo. They are not hot whatsoever (but I used to keep my room like a freezer, now that I have cpap that has changed). They are self-regulating as far as temp goes. They aren't cold like the other pillows that I currently have in my bed, like a buckwheat hulls pillow, for instance. I'm using one of those as a snuggly now and it stays cold all night, even underneath my covers, in the areas that my body isn't directly up against. By contrast, I use another silk pillow for my legs and its never startling cold (even if its cool to start out with) and it warms up just the right amount with my body heat. Maybe they just warm up lightening-fast compared to other pillows? I really don't know how to explain it, sorry!

Yet another option is to make one like fellow cpaper pjwalman did. It is in the shape of an 'H' - small and stuffed with whatever you choose. It should be used on top of another pillow.

I am sure buckwheat does degrade over time but the regenesis pillow is filled with buckwheat hulls (not grains!) and they are cleaned and free of dust.
