William, hope this helps answer your question:

When the airway is blocked or partially blocked as it is during an apnea or hypopnea event, the body must exert more effort to pull air into the lungs, causing a change in pressure and more blood to flow toward the heart. The heart interprets this increased blood flow as an alert that there's too much fluid in the system. It then sends a signal to the kidneys via a hormone, the essence of which is "we have too much fluid in here; it's time to dump some overboard." The body's way to dump liquid overboard is to urinate.

When the airway is clear, as it would be when Positive Airway Pressure is acting as a splint to keep it open, this whole chain of events isn't set in motion. As if there weren't already enough good reasons to treat your Sleep Apnea, this is one more. Save the trips to the bathroom and sleep easier at night.

I feel and the increase amount of trips to the bathroom is caused by the fact you're waking up to breath. And since you're somewhat awake then you feel like your bladder is full. The question is are we waking up to go to the bathroom or are we waking up to breath and then gor to the bathroom.

I've had no change in urinary habits since starting CPAP. Before CPAP I got up 0-1 times per night, and after 9 months of CPAP it is the same.

well, not exactly. It's not due to the lungs exerting more effort. It is in response to low O2 levels in the blood and the cascade of responses to those low O2 levels.

From a Dr. Umlauf: "...during episodes of sleep apnea, the soft structures in the throat relax and close off the airway, setting into motion a chain of physiological events. "Oxygen decreases, carbon dioxide increases, the blood becomes more acidic, the heart rate drops and blood vessels in the lung constrict," says Umlauf. "The body is alerted that something is very wrong. The sleeper must wake enough to reopen the airway. By this time, the heart is racing and experiences a false signal of fluid overload. The heart excretes a hormone-like protein that tells the body to get rid of sodium and water, resulting in nocturia."

Atrial natriuretic peptide (ANP) increases with sleep apnea . This increase is what drives the increased urination. It's basically a diuretic.
Succesful treatment reduces ANP and decreases nightime urination.

My husband was urinating 4-5x nightly and was diagnosed with a moderately enlarged prostate. He was given some medication which really didn't help much. Since being on cpap, he only gets up 1x nightly and sometimes not at all. So is it a coincidence that his prostate just shrunk at the same time he started cpap? I don't think so!

I agree with Dori. Before therapy, 2-3 times a night. After starting therapy, 0.

Before I was diagnosed, I would get up every other hour to visit the bathroom. Now since I have been on CPAP (just a bit over a year), I will get up once the entire night.

I would still get the ole prostate checked by a uroligist. The PSA test is not failsafe, but I wouldn't go over a year without one.

I know, I've had three operations on mine, including laser (which I would not recommend).

Thanks, guest, for that medical explanation.

Me too, before realizing I had apnea I used to think that my getting up every 1/2 hour or so was the result of having to pee all the time, just a symptom of getting older. Since I started CPAP I only get up once or not at all. I'm so happy I'm sleeping now. It's a whole new world.

To be clear, while it is great that Mike's prostate symptoms have abated, the increased urination caused by OSA (via the mechanism described above) is independent of the prostate. One should still get his prostate checked as recommended by his PCP.

Prostate cancer is not the funnest thing to do in your life. I have the T-Shirt.

Get a PSA test which is an imperfect but best available "pointer" to the symptoms of prostate cancer - Prostate Specific Antibody = PSA, or something like that.

Having been there, done that, my nighttime urination has decreased by about 1/3 since starting xPAP therapy. Give it some time to work, mine is only working on nighttime urination, (called nocturia, I think), and, very occasionally, feeling a bit better in the morning.

So, get on with some kind of therapy, give it some time, we are very complex organisms and each reacts differently to tweaking the system, And give thanks to the blower-folks who make it possible to wake up alive.

This forum is filled with good folks who will support you through most anything. Lean, they won't let you fall,
