new to cpap; worry that pressure is too low; air leakage

Post by skinnysleepapnea » Wed Aug 24, 2005 6:39 am

Hi, I'm new to cpap (2nd night at home-- 2 at sleep center).
I am worried that they have set my pressure too low or that leakage from front of mask is causing problems.
The leakage from front of mask (I have RESMED Ultra Mirage, cpap is respironics pro with cflex) is a definite roar on both inhale and exhale from area I believe around where it says CLICK that is a nose piece that covers the swivel joint that connects to the tube. I feel strong gust of air coming out of this whether I am inhaling or exhaling.
The sense that I have that the pressure is too low (of course the mask problem could explain it but I would like to know what you think about sensing the pressure is too low) is that
1) I am still waking up 3-4 times a night
2) I don't feel great in AM, rather I feel like I needed more air
3) feel sense of numbness just as I am waking up
4) take many deep breathes in the morning to feel better
Any advice will be appreciated,
sincerely yours Skinny

Skinny, welcome to cpaptalk.com! You're in exactly the right place to get some wisdom and solid advice!

From what you've said, first question is "Is your UltraMirage the Full Face version or the nasal version?" I've used the full face (FF) version for almost 8 months and have found that the swivel where the hose attaches to the mask is a definite leak potential! Sometimes, it would take 3 or 4 attempts to get a good seal. It's a hard place to find the leak, too, at least in my experience. It kind of whistled and was just hard to narrow down exactly where the whistling came from. Another thing, if you've had the mask apart, if you didn't snap the cushion in properly, you could be getting leaks from there. The cushion (at least on my FF mask) lets you know when it's seated with a soft click.

Could your pressure be too low due to a leak? I'm not a professional, a R/T or a doctor, so my advice is strictly from experience and probably should be ignored, but IMHO, short answer is yes. If the air from your CPAP is going out in places it shouldn't, then it isn't going where it should and yep, that could be a problem. It would probably be a good idea to take your entire setup (CPAP, humidifier, mask) back to the DME or R/T and have them take a look.

Finally: This therapy WORKS!!! For some, it works almost immediately, for others, it takes a while. Hang in there.

skinny -
I have Resmed Ultra Mirage II which is very similar to yours in that it has that cover with the "click" on it. The first 2 nites there was a LOT of air and noise coming out of that area. On the third nite, I found out by mistake that it takes as good amount of pressure to seat that curved piece properly.
If that curved piece flicks off easily it's not seated correctly and that would amount to a BIG leak. I takes a good tug to get it off if it's right.
When the curved piece was seated, the noise was almost eliminated but the air still flows out some because that is the exhaust port and I get a continual flow from it. And then I felt it was helping me the way it should.
Hoping this helps you

2nd night, don't be shocked ur not feeling great yet.
The air gust in the front is the exhilation port-- don't cover that or Co2 build up will cause problem

- if you think the air leak is too much, set the alarm on CPAP unit. it will sound when leak is greater than it should be

- there should be a card on the side of ur machine. You can take that to ur dme to be read. It will show of OSA still present.

Good luck!

Do you have the card reader so you can read your own data? If not, you can easily purchase one at cpap.com or other online sites. It will eliminate having to bring the card to anyone else to be downloaded. Saves you the time and expense of a trip to your DME or sleep doc. Also, adjusting the pressure is simple to do yourself.

OOPS! My bad!

What a bummer, though..... a machine with software and nothing to look at but compliance.... what a gyp!!!
