anyone on a lower pressure than titration?
hey all
i had been titrated at 6cm jan. 2007, but increased my pressure to 8.5 in sept....
then i had a new study done in january 2008, where they settled on 10...
doc. said to try 9.5 for a while, which i did...
i've been feeling like hell since january...at the study i was mouth leaking, which hadn't been an issue at 8.5 cm...
since i increased to 9.5 i've been mouthleaking like crazy...so i've been trying fullface masks, with no success so far...but i'm about to trade my fp431 in one of these days (when i get up the energy to go to the dme, lol)...
since i was still feeling so rotten, i changed my pressure to 10, but after a week on 10 i was feeling worse than ever, pretty close to my pre-cpap condition, and mouth leaking even more (been trying to make a chin strap work, but it's just so uncomfortable after a year on the headrest which is like going commando )...
so last night i reduced the pressure to 9 cm, and my numbers were great this morning (ahi 0.9) with a great leak line, and no mouth leaking at all; i know i have to give it a week or two, but this seems encouraging...
so whaddya think? anyone have success with a lower pressure than the study suggested?
thanks
sharon

Sharon,
You KNOW I was.......titrated to 16......doctor prescribed 18......and I started myself out at 10......am now at 12. AHI has been essentially identical between 10 and 12, but the snore index is lower at 12.

I was titrated at 12, the aerophagia was unbearable to the pointy that I would wake up with severe cramping. After enduring this for a week I called my dr and he prescribed an auto machine. My settings are between 8-12 I have never hit 12 and only hit 11 once for about 6 minutes one night. My AHI ranges between 3.5 and 0.5.

i barely know what I'M doing these days, never mind anyone else

lower than titration
Hi Sharon,
I started at a pressure of 15, begged to have it dropped, so it went to 13. Begged for an auto, and found that my 90% was sometimes 11 sometimes 12, and the lower limit was 7 and I was there a lot. Managed to convert to auto bi-pap with pressures of 9/12. I get 0.0 AHI several times a week like that. The bi-pap makes it easier for me to breathe. That 9 feels like no pressure at all and it keeps the apneas away and almost all hypopneas.
Just goes to show you that when you are home you sleep differently than you do in the sleep lab.
If you can get the data, you should be able to judge for yourself how it is working over time. How you feel is definitely part of it.
Good luck. You can always find help here.

A trick answer, I was titrated, accurately I believe, at 18 and am now using 8.5 and getting AHI near zero.
The trick? I am wearing a backpack to force myself to sleep on my side where the pressure requirements are much lower.
When a sleep lab titrates someone at such a high pressure (18 in my case), why would the doctor fail to suggest you might need a much lower pressure if you slept on your sides?
Thank goodness for home data capability and software.

Hey Sharon,
I was titrated at 5 and am now at 10.....with reading all the posts over the past year, I've kind of come to the conclusion that the titrated pressure may be right may be wrong and reality is what the data and your body tells you.....I tried it higher but 10 seems to work best for me.....
If you feel a whole lot better at 9 -- yee ha!!! But you're right. Keep it there for at least a week and evaluate then (I do it for a month unless there is something terribly wrong)......
It sounds like you're on a good track!!!! That's great!!!!
Great new avatar by the by.......
take care!!
cheers

that's just the kind of input i was looking for!
unfortunately i don't have an auto, but i do have data collection and i think i might be on the right track for once

Try this setting for 1 night:
Auto:Min pressure =6.0
Auto:Max pressure =9.0
You could optionally try Auto:Min=6.5 so Cflex/Aflex was enabled (those are disabled below 6.5 cm pressure).
Note: the Auto:Max pressure suggested is critical for this test, if you are not going to drop it down to "limit" pressure exposure, don't bother with the test. Remstar will probe above that setting by 1 cm anyway.
If you feel a little bit better the next day, you may want to lower the Auto:Max=8.0 (again Remstar will still probe 1 cm above that).
