I just got the Deluxe chinstrap because my leak rate is too high and I have not resulted very much from the treatment after more than a year. I wore it for the first time last night. My AHI was higher than normal. What gives? Also, how tight should I wear it? Should I be able to move my jaw at all? What is the sccop about chinstraps ruining your jaw? It was uncomfortable in the fact that it smooshed my checks and chin in (chubbyness). And this is on top of the fact that my mask sits too low on my mouth and I have some pressure there too. (I already wear the petite.) So my mouth was a smashed mess.

I wouldn't give you $.29 cents for that Deluxe chinstrap, might as well just wrap your head with a cardboard box. But keep the large strap, can probably use it to pull your car out of the ditch if you slide off the road.
Chinstraps generally don't work (while they sell a lot of them), they don't work because they pull the mandible back especially the ones that cup the chin and the crown of the head, this the exact opposite of what you want to do to open your airway.
If you observe the x-rays on a MMA procedure, you'll understand what I mean. Your AHI went up most likely because the chinstrap pulled your mandible back making the airway at the back of your throat narrower.
You can overcome mouth breathing if you address chronic nasal congestion and practice holding your tongue into the hard palate while awake and keeping an eye on your leak data.
But you cannot beat the efficacy of nasal therapy.

SD is right izzyb, you should try and train yourself to keep your tongue planted on the roof of your mouth at all times and learn to do nasal saline irrigations on a regular basis like brushing your teeth.

I have a different twist on the chinstrap problem, and any insight is appreciated. I was a buck toothed youth, for whom braces resulted in considerable movement of my upper teeth to a normal position. Never had any tooth on tooth impact problems prior to CPAP while sleeping.
On 10 cmH20 CPAP I am awakened with lower incisor impact upon the back of my upper incisors. I use (not too successfully) a Respironics Premium Chinstrap to assist in stopping my incisor impact.
Anyone know an alternate approach that might work? Any insight appreciated... CPAP therapy is working well, except for this tooth impact issue.

Respironics Premium Chinstrap
I really like the Respironics Premium Chinstrap:

It's very different from the Respironics Deluxe Chinstrap. It stays on my head the whole night and is actually quite comfortable. My AHI is always lower if I use the chinstrap, but everyone is different!

I tried the chinstrap for two nights in a row and that is it for me. After reading about the damage it can do to your jaw and the fact that it did smash my mouth in, I don't want to risk that. Not to mention my high AHI while using it. It seemed to make the problem worse. My DME rep called me to check on how the chinstrap was working. We talked for a while and I am going in on Thursday with my gear so she can help me see if it is fitting correctly, etc. We are going to try an Ultra Mirage full face (they have one I can try on) and I might change to that. It also sounds like maybe my expectations are too high. Maybe I am never going to feel fantastic. I am not sure if I want to go this route, but I don't know what else to do. I did try mouth taping for a couple of nights and it did not improve my leak rate. I don't have any confirmation from anyone, my rep or the boards, on whether or not a leak rate of around 40 is too high...the chart says 32. So maybe that is okay.

If the leak rate is regularly going much above 40, I'd want to do something about it for sure. But if it's staying "around" 40, 41 (or the high 30's) you're probably doing "ok" with leaks and are still getting effective treatment.
I'd keep working on finding a way to get the leak rate down, but it doesn't sound like the rate you're getting is a treatment wrecker. Downside is that your increased leak rate might be making the machine use more pressure on you that it would have to if the leaks were under better control.

I wear a snorban dental device with my chin strap. I also tape. The snorban keeps my teeth from grinding and holds my jaw in position so it doesnt slide back and restrict the airway. Some people may not feel comfortable wearing all this. It takes time to get used to. A full face mask is another way to maintain cpap pressure if you mouth breath or mouth leak. I don't know what will be best for you. Eventually you should find what works for you if you stay with it.

I have another take on the chin strap thing. I tape but that alone is not enough as I am a heck of a mouth breather if I let myself. I wrap an ace bandage fairly loosly around my head from chin to top of head. I look like hell but it is just enough support to work with the tape.

I forgot to add that someone at my DME's office lost all of my data prior to January 24th (that is when I took my card in to have my pressure changed). I was assured that my old data would be downloaded. I found this out when I took my card in about a week ago to get the rep to look at my data. She told me they had nothing prior to that date, but she is looking for it. I am not happy.
But anyway, the info that I got from her regarding my data is that it is showing toward the early morning hours I am snoring nearly every morning. I can't believe I forgot to put this in my earlier post. Well, yeah I can believe it. I still don't function as well as I wish I did. Maybe not even sleep therapy can cure that.
Anymore thoughts on the snoring? Is that consistent with possible mouth leaking?
