Apap question

Post by bigguy42431 » Fri Mar 28, 2008 8:37 am

I have been using a rental REMstsr Plus m series with H/H, set at 14 with c-flex set at 2 for the past 9 mos. It has helped my sleeping alot but still think it could be better.
I just ordered a m series APAP with A-flex and H/H bundle from cpap.com and would like any commits from users on where I should start with settings.

My titrated pressure on CPAP is 10. The board member here suggested that I use 9-13 and recently said maybe a 8-14. I actually set it for 8.5-14 and all is well. Just to give you an idea of what the ranges can be.

I love my A/Flex by the way.

First thing I'd suggest is to get the smartcard, reader and software for your machine. That way you can really measure what's working and what's not by looking at how various settings effect your AHI.

I'd set it for auto 10min 20 max a-flex @ 2. If you have the software, you can see where your sweet spot is. If you find for example that you spen most of the night above 12, set your min for 12, that way the auto will have less adjustment to make, and get to the required pressure faster.

As I said, you may need to move the lower number up (or down), but I wouldn't mess with the top number unless you find it too hard to keep your mask sealed above a certain pressure. I've had my best luck with my max set @ 14.5 because when I get to 15 it's almost impossible for me to keep a good seal.

If you need more information keep reading this forum, or ask more questions, everything you need to be successful is here.

Yep.......get the reader and software.
Since you have absolutely NO data from your current machine, I would set up your new Auto EXACTLY like your current machine. That way, you can see what's happening with your existing configuration and settings.
You'll be able to see whether you're leaking, having events and all the details to see how you're doing. In other words......establish a "baseline".

Put the encore viewer software and card reader on order. Does this software run ok on windows vista home premium. I have another computer with XP on it but would like to use the vista machine.

The 10 - 20 KHENAULT suggested would be fine to try.

I'd probably set the minimum up a little higher if I had been prescribed a straight cpap pressure of 14. At first I'd probably see how it felt with the minimum pressure at 12 and max at 20.

I'd set A-flex at 3. Or whatever A-Flex setting feels comfortable and smooth to you.

Den's suggestion to set it in cpap mode with C-flex and use it just like your previous machine, in order to methodically get baseline data, is fine too. I just wouldn't have the patience to wait! I'd want to see what the new puppy feels like in auto mode.

Given a previous prescribed straight pressure of 14, the settings I'd try for the M series Auto with A-flex would be:

Therapy Mode Auto
Comfort Type A-Flex
Auto: Max 20
Auto: Min 12
A-Flex Setting 3
Autoramp Time 0:00
Mask Alert Feature OFF
Auto OFF Feature OFF
Split Night Time Off
SHOW AHI/Leak ON
Patient Reminder
OFF

As the others said, after you get the software and can do your own downloads to see the detailed data, you can tweak things. Mainly, tweaking the MINIMUM pressure. The maximum pressure can probably always be left where it is ... at 20. Unused pressure up there doesn't really matter since it won't be used anyway.

Getting the MINIMUM pressure set right, which usually means having the minimum set high enough to prevent most events right from the get-go, is usually the key to effective autopap treatment for most people, imho.

Happily, autopaps can be set to straight cpap mode, if you find that suits you better. Two machines in one.

I have Windows Vista Home Premium on my computer and Encore Viewer runs just fine - no problems at all. Enjoy your new machine. It's about the most versitile available right now!
