Daytime Sleepiness

I have been on CPAP for about 2 month and I do feel a lot better. However I am sill experiencing times during the day when I have some sleepiness. With CPAP my AHI is never more than 1.2. Does anyone have an idea about what might be going on.

Your body is probably still trying to catch up on all the sleep you've missed before xpap. With such a good AHI, I wouldn't worry. Just give yourself time to get caught up on your sleep, it can take a while.

If you are waking frequently to handle leaks, hose location or other agrivations, this can cause inefficient sleep the machine cannot sense.

Also, large leaks mean the machine cannot sense events during that time.

No significant leaks and I only wake up once during the night to use the bathroom and never have to touch the mask. Leak rate is usually less than 1lpm.

As Brenda said- give it a good long time to work- it will take awhile to get our bodies back to normal- sounds like your machine ios working good, and no significant problems, so just give it another couple of months at least. Also, waking up even once at night, when coupled with the fact that it's been happenin g for a very long time, is enough to cause sleepiness problems during day. Most healthy people can handle being awakened without any problems, but apnea patients have often been sleep deprived for so long that any little itneruptions are magnified andwe suffer during hte day for it.

You may have another sleep disorder like narcolepsy or excessive daytime sleepiness EDS. Most people with narcolepsy are diagnosed with OSA first. You don't have to fall asleep driving or crash on the fall unconscious to have narcolepsy. So be sure to see you sleep doc if you don't feel better soon. Maybe some tweaking needs to be done.
