After 7 years of good CPAP use, I FINALLY went for a new sleep study.
Yes, I am still moderate - severe, but I want one of those small Resperonics as I travel frequently.
My sleep doc put in the notes (that the evil insurance company will surely see) that I use my CPAP almost every night.
He is prescribing a new machine for me, but I am not hopeful...
How can I get BCBS to cover a new machine for me?
You could contact BILLMYINSURANCE.COM (a subsidiary of CPAP.COM) and see what they can do......they apparently work with BCBS.
It's also been reported here that those using BCBS can get reimbursed for out-of-pocket purchases if you decide to purchase online (like through CPAP.COM). In this scenario, you purchase the equipment and then submit an invoice from yourself to BCBS and they send you a check.
You would also need to contact them to pre-approve this arrangement.

Since I also use BCBS I was wondering why you think you'll have problems.
I've only been using CPAP about 5 months, but when I asked BCBS about their replacement policies (masks, machines, etc.) they told me that they replace masks every 3 months, hoses and filters as needed, and machines every 3 years. I have no idea what they will do as far as payment for my machine. My DME says with BCBS it will be a purchase not a rental, but as they lost my info after they delivered my machine and it took 2 phone calls from me and 4 1/2 months to finally get it billed I'm not sure how everything will work out.
At 7 years though, definately sounds like you're more than due for a replacement machine.
Have you had problems with them before?

You should go back over SEVERAL forums and learn the advantages of the APAP over CPAP.
Listen to the arguments FOR APAP and insist on one from the doc.
Learn that APAP is covered even with a CPCP rx ar cpap.com and many others.
Inform yourself and know YOU are in charge of your treatment.
Or be non assertive and let them give you what THEY want.
can you say cheapest?

You are in for a rude surprise if you have BCBS of Oklahoma. They have no replacement "schedule" Everything is sent to the claims department for "medically necessary" or not. I have this in voicemail from their customer service department.
Even the 2 DME's I checked with can't tell if a claim will be approved.
