BiPAP not picking up inspiration sometimes

Hi! I'm migrating from CPAP to BiPAP and have been struggling with the device not picking up my inspiration and not triggering the IPAP often. Could this be a breathing problem or a device issue? I initially noticed a clogged nostril but even after decongestion it kept happening.

Knowing brand and model of machine might be helpful to any responders.

Many possibilities. Please state machine and mask models, mode setting, pressure settings including ramp, ...

If you don't know how to get into the clinical menu to check the settings, find a youtube video for your model machine explaining it.

Here is my setup:

Device: BPAP AUTO GII BMC T-20A Ser. (https://en.bmc-medical.com/products/npa ... t-25a.html)
Mode: S
Mask: Phillips Nasal DreamWear
EPAP: 6 cmH2O
IPAP: 10.5 cmH2O
Ramp: 10min
I Sense: 8
E Sense: 8
Hose length: ~1.75m (no bacterial filter)

P.S.: BTW, the hose is passing through a hanger on the wall so that it does not get too lengthy on the bed.

Is it the AirCurve 10V? The model name should be on the front of the machine. https://www.cpap.com/productpage/resmed ... el-machine

As a general rule, ResMed bilevels have a setting in the clinician menu for adjusting trigger sensitivity to help it recognize when you breathe in. There should also be a sensitivity adjustment for cycle, when you breathe out.
"Under normal conditions, the device triggers (initiates IPAP) and cycles (terminates IPAP and changes to EPAP) as it senses the change in patient flow. . . . I believe most users do well with "medium" for both, although a few users are more comfortable with changing those for increased or decreased sensitivity.

My device has sensitivities ranging from 1 to 8. I have played with both "I Sensitivity" and "E Sensitivity" for many days. Both are at maximum lately (=8). But still, every once in a while when I finish breathing out and start to breathe in, it does not switch the pressure and it gets hard to breathe in.

How long is the hose?
Do you have anything in the hose line like a bacterial filter?

Hose is about 1.7-1.8m. No bacterial filter.

Just wanted to share that I've noticed that this was happening during ramp only. I've made 2 changes that seem to not trigger the issue anymore for the last 2 nights:
1) Increased the ramp time from 10min to 15min.
2) Reduced the "E Sensibility" from 8 (max) to 7.

If the issue comes back I will update in the thread.
