Do I need to change my CPAP

Post by FossilMind » Tue Aug 23, 2022 10:58 am

Hi all.

My first post here but since February have been using a Resmed Airsense 11 with ResMed AirFit F20 full face mask.
I have been struggling to bring my results down until I found that a MAD (Mandibular Advancement Device) could help.
I got one and saw a big difference.

OSCAR DATA DATE RANGE AHI OA H UA CA 95FL RERA
Global treatment 2/5/2022 - Now X 2.51 0.97 1.35 0.01 0.18 0.33 1.75
Before MAD 2/5/2022 - 6/6/2022 Y 3.47 1.36 1.89 0.01 0.21 0.37 2.25
After MAD 6/7/2022 - Now Z 0.91 0.31 0.47 0.01 0.13 0.28 0.93
(Z - Y) / Y -74% -77% -75% 0% -38% -24% -59%

I still have episodes of hi AHI and I am wondering if a change for a bi-level machine would improve my results.

The attachment is of one bad night.

In case it would be helpful for you to have a more complete picture of my situation, I have a ZIP of all Oscar data that you can download here:

I thank you in advance for any comment you would make.

What do you consider a "high" AHI night?

What is this data point referring to???
(Z - Y) / Y -74% -77% -75% 0% -38% -24% -59%

Instead of posting a string of numbers or a zip file of more numbers, post some OSCAR graphs.

Look around, that's what people are asked to do. That's what commenters like to work with.

I would very much like to post a view of OSCAR.
I followed the instructions from
Re: How to post images for review

I see the file name in the Attachments window and when I click on status (yellow triangle) I get this message:

Failed to move uploaded file.

Is there a limit on the weight of such file ?
The file I tried to upload is a PNG, taken with Windows Snipping tool.
It's weight is 1,172 KB.

Sorry for the figures all screwed up, I was trying to post an Excel table.

So, what can I do next ?

There is a file size limitation to attachments here...1 MB max.

And there is an overall general limitation for attached file storage for the forum (everyone's attachments) that very well could be already maxed out.

Currently the board (not individuals) has reached its capacity limit.

Until this is resolved, images can be stored offsite and then linked here. imgur.com is a popular host.

 sorry for the wrong turn. should've remembered the inability of the forum to take attachments.

forum friend dog slobber has the right of it. imgur.com is free and is really the way to go.

good luck!

I change the view of Oscar as per your request.

I have added a graph showing the evolution of different indexes.

I hope it will help.

that's quite some improvement, based solely on your ahi! you went from 5.65 to .70. this is very encouraging as you are trending in the right direction. keep at it and let's see how it goes proceeding forward.

imho, there is no benefit to be gained in going to a bilevel machine. you haven't begun to really get the most out of the machine you have.

patience is something that needs to come into play. i'd advise staying the course for now.

it seems that the MAD is helping though i'm uncertain as to how. maybe i don't understand the device well enough to comment.

by the way, how are you feeling after a night of sleep?

From zonker:

The MAD is just like a mouth guard for hockey players except that it is in two pieces and it pushes the lower jaw forward.
What it does is opening the airway. That's my understanding after reading online multiple articles about apnea.
From zonker:

I am getting to 7 months of treatment with the last 3 months using the MAD.
I was thinking that was long enough to evaluate the success of the treatment. Seems not so ???
From zonker:
by the way, how are you feeling after a night of sleep?
So, that is why I am checking everything.
I did not see much change from before the CPAP in the way I feel after a night sleep.
If I sit down in the day, I often go to sleep.
My sleep is always cut in two or three sessions.

from my point of view and from i can see here, the treatment from your current machine AND the device are bringing your ahi down. sometimes, we have to look at other areas for overall sleep improvement. getting low numbers doesn't always guarantee a good night of sleep.

are you on any medications? if so, what?

also, google "sleep hygiene". it's a terrible name but it's a list of things that you can change in your everyday life that can help you get a better night of rest.

My personal opinion is that your hours of use leave a lot to be desired.
From a quick look at the usage graph you just posted it looks like the bulk of the nights your number of hours using the machine are quite low. You have many more nights with under 4 hours than 6 to 8 hours.

I feel like total dog crap with less than 6 hours and I know there will be a nap in my future. I feel my best when I can get 7 plus hours with no fragmentation into multiple sessions.

So the fact that you are saying you haven't seen much change in how you feel doesn't surprise me one bit.
You need more sleep using the machine....now why you aren't getting it is a subject for a different discussion.
