ear pressure problems & appliance to keep jaw forward

I'm new to cpap (one month) and my doctor and I agreed that I should stop using it (at least for the time being) because of middle ear problems.

As I continued to use the machine and even lower the pressure, the worse my ears got. I've always had problems flying because of intense ear pain.

My doctor thinks I may need an appliance to keep my jaw forward - would you have any information on this also?

he was probably talking about a dental device for treatment of OSA.
