Broken Tooth

Post by bierhere » Tue Dec 29, 2020 1:07 pm

What strategies have you employed when you have a tooth problem. I have a broken tooth that needs extraction and I'm waiting for the oral surgeon to have an opening. I have a full face mask and after having CPAP on for 1-2 hours, it is creating sharp pain and wakes me up. I'm alternating between Advil/Tylenol and using heat/ice, which is getting me through the day with some discomfort, but sleeping is total torture right now.

Without CPAP, I can't sleep either. I have an AHI of 67 and low oxygen stats and basically wake up every 10-15 mins without a PAP on.

I turned down the pressure on my PAP which helps a little, but not much. I ordered a nasal mask/chin strap as a temporary approach, not sure it will work, but figured was worth a try. It arrives tomorrow. Any alternative suggestions?

Ouch! I'm sorry. That's a miserable way to spend a holiday week.

Is the mask pressing on the nerve that's irritated? If that's the case, hopefully the nasal mask will help.

Is it the cold air? You might turn up the temperature of your humidifier to see if that helps.

Have you tried any over the counter treatments for tooth pain? You might call the oral surgeon to see what he/she recommend while you're waiting.

A pharmacist is likely to have something for that - I'd definitely ask one (or 2).

Which tooth is broken (https://dentalimplants-usa.com/bone-and ... numbering/) and which model mask are you using?

Update my equipment profile, but I have a DreamStation APAP with Humidifier, ResMed F20 Full Face Mask. I also own a ResMed AirMini for travel, though it doesn't get used much anymore given COVID.

Tooth in question is #31, bottom right molar. It feels like the "Air" from the PAP is the problem not the mask pressing on the nerve. I have loosened the headgear on the bottom and tighten top and left side of mask to compensate. I could look for my old Mirage Quattro mask to see if it sits better. I might still have the last one.

I was curious if humidity is good or bad? I was thinking it would help, but it does bring more bacteria with it potentially.

My dentist recommended alternating between Advil / Tylenol every 3 hours at prescription type dosages. This works to a point, but even awake, I'm having to use ice / heating pads to control the pain at times. My dentist was not willing to do the extraction because I have three major diseases ( Crohn's disease, diabetes type 2, and latent TB ) in addition to the sleep apnea. I called two oral surgeons, one quoted me Jan 13th, the other March 30th, but the March 30 one I've used in the past, and they understood the urgency and put me on the cancellation list. They said I should hear back in a couple more days. So, it could be 1-2 more weeks at a minimum.

I'm not sure if I have ever used it but it has been around a long time. Would Anbesol help? Tooth pain is no fun.

I had a broken tooth once. It was awful, partially because an orthodontist thought it wasn't broken and tried to do a root canal, but never could because the pain could not be blocked.
I don't use a full face mask so I didn't have the problem with cpap that you are. Advil didn't touch it. I have bad back problems and this access to much stronger medications, still didn't touch it.
Anbisol can't touch it either. If your break is above the gum line then maybe it could work, mine was at the root.
A hard to find option is clove oil. It was the most effective treatment i found, but it wears off quickly. It is said to be about as strong as benzocaine, but for me it was better.
You don't want too swallow too much of it because it will upset your stomach. Same is true for benzocaine, but that will also build up in the blood and cause bad issues.
I "over- used" the clove oil, partially because pain and research don't go together, and partially because the pain was so debilitating.
Lastly, keep calling and find a doctor that can do it as an emergency. That's the only thing that will help in the end.
Best of luck ... you have my sincere sympathies!

There is no evidence that a CPAP humidifier delivers bacteria to the user.

Mouths should be kept moist. Chronic dry mouth leads to caries and gum disease.

I hope you get a call about a cancellation soon.

I have used cloves from the grocery store spice aisle. Just tuck 1, 2, or 3 between the cheek and gum. Let it sit all day/nite. The cloves are hard n sharp so it may help to soften it up some in warm saline or water. HTH

Received ResMed N30 mask and a chin strap yesterday. Tried in the afternoon and it felt very difficult to breath through he nose since. I could not push air easily enough, like many people have a deviated septum that has never been fixed. So, I ran out to the drug store for some breath right strips.

With a breath right strip, I was able to use the nasal mask and get a full night of sleep with little to no pain. It was heaven. I did keep the CPAP pressure down a bit to help with the transition. Not sure I'd want to use this permanently, but at least its a short term workaround. :D

The best solution is prevention. (cheaper, too)
Regular dental care beats trying to sleep with a demon in your mouth.
I have noticed that having my teeth cleaned at 6 month intervals beats the heck out of waiting longer.
If I have been a good froggie (regular brushing), there is hardly any discomfort at all.
Not much help for the OP (sorry), but for others--something to think about.

It's been nearly a whole year since this thread was started.
I really hope the OP saw a dentist.
And continues to get regular preventive treatment.
