BiPAP settings

Post by thellra2 » Thu Jul 23, 2020 8:19 am

I recently acquired a S10 vAuto Bilevel.

Previously I'd been using a APAP machine with the settings dialed into 17-20, from time to time I'd hit 20, and started to notice a few clear airway events, causing me to want to give a Bipap a shot.

What would be some sane settings to start with?

There's no such thing as "sane" settings, for everyone. When I moved from APAP to BiLevel, I used comparable settings but took advantage of the extra pressure support.

Without knowing your exact settings, especially any exhale relief, we can't suggest equivalent settings.

My settings on APAP were:Pressure:
11.2 - 20
EPR: 3

I set my VAuto to:
PS: 4
Min EPAP: 8.2
Max IPAP: 25

For you:
I would set:
PS:4
Min EPAP: 13
Max IPAP: 25

Then watch graphs and adjust.

DS has given you a good place to start. Let us know how those settings work for you.

A couple of additional thoughts.

First, a bilevel machine won't help you with CAs -- but the good news is that "a few" of them is no cause for concern.

Second, to continue with your self-titration, you would benefit greatly from using the Oscar software:

https://www.sleepfiles.com/OSCAR/

For this, you'll need an SD card in your machine, plus a laptop or desktop computer, plus an SD card reader, either built into your computer or purchased as a plug-in. Once you're set up, you'll be able to post charts showing in detail what's going on. That will allow people to make informed suggestions about how to dial in your settings so that they are optimal for you. Let us know if you have questions about any of this.
Oscar software is available at https://www.sleepfiles.com/OSCAR/

a few points, that machine is "not" a BiPAP™, nowhere on the machine, box, manual, or marketing materials will you find the word 'BiPAP™', because that is a marketing term of Philips Respironics.

Second, and most worrying, if you're thinking you have central apneas that are being cased by an APAP, then the VAuto will do nothing but exacerbate those.

I'm talking a few a month on OSCAR for centrals.

The big selling point for me was being able to go over 20 pressure if needed. Whenever I get at all sick, or have had something to drink, the apap seems to go straight to 20.

I didn't like the airhammering I got on the apap when it had the exhilation relief, on the Vauto it doesn't seem to do that, it's pretty gradual.

I'll fuss with it for a week or two and come back with some graphs.

Unfortunately it looks like the 'new' machine ate the SD card data, so I think I'll be starting from scratch.

Word if warning, if you jailbreak your S10, don't have the SD card in on its first boot on a new firmware.

What APAP did you have?

I have a dream station, and had a S10 apap.

First night's sleep on the Vauto just happened and holy cow, I had dreams, I feel great.

I ended up doing inspiratory of 17 to match what I had and I think 10 for the normal support, machine reported 0 for ahi

Dreams are a sign of disrupted sleep. If you sleep through them, you don't remember them.

Found out last night if you set the PS to 10 you can hyperventilate yourself...

Set the PS to 5 and the min Epap to 12 so the Ipap matched my old APAP settings and had a great nights sleep, woke up (without dreams)

Think I'm going to not fuss with it for a few days and do an oscar pull on Monday or Tuesday.
