If money were not a problem or insurance didn't care what device you bought (ha, As if!)

What machine would you go for? CPAP, BIPAP, ???

And what model?

And WHY?

I'd get the Resmed AirCurve 10 VAuto.

It's not a "bipap" because bipap is a product only made by Philips Respironics.

It's a "bilevel", (Resmed calls their bilevels "Aircurve" or in the older machines, "VPAP", Respironics calls their bilevel machines "bipap".

I'd get it, because it's the most customizable, most comfortable and responsive machine.

It's like a Resmed Airsense 10 Autoset, but better, more features, more potential customization to make it as good as possible for you.

Not all that much price difference--but the features are well worth it.
If price is an object, go for gently used--often cheaper than brand new OK--sort of.

As an additional question... I think if one were to buy a used machine, wouldn't it be best to get a new headgear and whatever mask/nasal pillow/whatever for one's self? Do all machines take the same hose fitting or have the manufacturers decided to go with no common standard, just to make people buy their stuff and not be able to use some other brand?

Just get a new headgear/mask when the old one is worn, or if you want to try something different. (Do keep a backup mask and hose - you never know what might happen.)

I would get exactly the machine I already have--ResMed Autoset. It treats my apnea perfectly, it's quiet, comfortable, works great. Personally I have no need for a fancier machine.

I don't understand what buying a used machine and getting a new headgear/mask have to do with one another. You can do either one without the other.

My concern would be if the used machine would come with headgear and whatever "cooties" might come with that. I was always taught to not share a hair comb, I think headgear might fall in the same category reasoning. The other concern would be, will my present headgear hose fit the new/used machine. I like what I have for headgear, but my CPAP machine is 23 years old and I am concerned with its longevity potential. I just wonder what is the "best" machine to get (and I realize that is a subjective assessment as to "Best"). Just getting opinions. I will say that the 'used' market seems to be the place to look. The price of new machines is, well... WHOA! (Personally, I think the fact that insurance exists has caused medical things, from drugs to procedures to machines, etc. to be higher than it would be if people had to pay out of pocket for them.)

If you're a fanatical, unrealistic germophobe, then yes, new hose, headgear, mask, etc.

Otherwise, just wash them... there's instructions in the back of all the mask manuals for 'reprocessing' between patients in a clinical environment.

With a couple limited exceptions, all masks fit all hoses and all (except heated) hoses fit all machines.

Cooties do exist, but they are not nearly as common or vexing as many people think.
And you can get them from your children, or grandchildren (head lice)

What I have now...the Resmed Aircurve 10 ASV. You spend a third of your life sleeping and it impacts the remaining 2/3's; so I spend what is necessary to get a good sleep.
