Illinois Medicaid replacement schedule?

As is apparently my M.O., I have managed to misplace the sheet that discusses when I can my supplies replaced - and it’s on the weekend, so I can’t just call the people who manage my Medicaid plan or the DMS they use. I know it seems like it changed last year, as I was able to get certain supplies like the disposable filters way more often and then last year they cut way back.

If anyone here is in Illinois and can tell me the Medicaid replacement schedule for the following supplies, I’d appreciate it:

Auto-CPAP machine
Heated Humidifier
Water Chamber
Disposable and Non-Disposable Filters
Headgear (for FFM)
Full Face Mask Assembly & Cushions
