AHI at home much higher than while using CPAP during sleep study

Hello,

Newbie here, I wanted to get some insight on my experience to date. I was diagnosed with severe sleep apnea 1.5 years ago with an AHI of 37.9. During my split-night test, a pressure setting of 8 cmH2O was able to get my AHI down to 1.1. (See results below).

I only recently discovered CPAPtalk.com and sleepyhead, and so I've been pouring over my data. The average AHI over the first 15 months of usage was 7.85. The lowest AHI I've ever seen using these settings was 3.27. Here is a day where I got above average duration of sleep and the AHI was 13.91.

I discovered through this forum that although the Rx was for a fixed 8 cmH2O, I could change the setting and that there was an auto function. Right now I have it on auto, with a min of 9, max 20, and am still getting an average AHI of 7.5.

One thing that I am noticing in the data is a higher CA rate than during the sleep study, which won't respond to the CPAP, but I'm not sure why I'm getting this. Overall, I'm not sure if I feel more well rested than before, but I'm sure much of that is due to overall lack of sleep (demanding job, new parent, etc.), and I'm working on sleep hygiene. I've used the F&P Eson mask (what was used during the initial sleep study) and the Philips Nuance Pro nasal pillows, and with both I do sometimes find myself fiddling with it in the middle of the night. I have yet to do a follow-up sleep study since the initial split-night. I know 7.5 is better than 37.9 but I think I can do better. Perhaps I'm just jealous of all the 0-1 AHIs I see posted here, and I figure that was once possible with me. But I'm also weary of doing too much dial-a-wingin too. Any suggestions? Thanks.

The ClearAirway/Central apneas might be nothing more than secondary to waking up fiddling with the mask (since you report doing that) and not really indicative of anything but maybe poor sleep quality and multiple arousals/awakenings.
Go here and learn how to evaluate if the flagged event was real as in you were really asleep or if it is a sort of false positive flag.

If the event is flagged when you aren't asleep...it doesn't count and nothing we can do about it except maybe try to fix whatever is causing the poor sleep.

Not long ago I had a night with AHI over 3 which is unusually high for me. Nice mixture of Centrals, OAs and hyponeas...not a single one of any of them was the real deal. They were all related to arousals/awakenings...and it didn't surprise me because I knew it was a crappy night because my back hurt like hell and I knew I had a lot of awakenings tossing and turning trying to ease the pain.

If your OAs/hyponeas/snores/FLs seem a little numerous...more minimum is needed. For some reason your machine doesn't want to go very high so if it were me...I would try 10 cm minimum...you can leave the 20 max...I don't think it's ever going to go anywhere near it but it doesn't hurt to leave it at 20. It becomes a moot point really if it doesn't go higher.
The minimum is your more critical setting.

I forgot....if the centrals appear to be real...try turning your Flex exhale relief setting off and see what happens.
