Machine Replacements?

I know its a ball park but does anyone know or had the situation on how often insurance pays for a replacement machine? I am on a 10 month rental through Apria(Yuck)! I have blue cross blue shield of indiana. Any experiences with that?

I have BC/BS of Western PA. (Highmark). Check with your insurance company. MY BC?BS recently sent me their policy (in writing!) on replacements as well as repair and replacement. Essentially, my folks will cover anything that has a CMN (certificate of medical necessity). Your Dr can send that in.

I have BCBS of CA and was told the same by BCBS as dataq1 about a replacement. I obtained the statement of medical necessity from my dr., had it faxed to the insurance company, obtained the pre-authorization, got the machine and the insurance company paid with no problems.
