EPAP 90% maxing out.

Post by RestlessinWoolley » Thu Jan 04, 2018 12:43 am

I have my one year follow up with the sleep doctor on Monday. And a tentative Sleep Assessment retest on the following Friday. My centrals have been really weird lately and the doc is concerned. Originally my pressures were as below 6/12 on ASV, max pressure was a 24 now open, epap min was 6/10 now 6/15. For a week or more with it set at 10 I was maxing it out but my AHI over a 7-8 hour night has averaged 1.5 AHI. She bumped it up to 12 and again for a few nights I maxed it out and my lungs have been hurting. She did bump it up to 15 till i see her and Friday I had a 7.0 is my average Epap, Sat and Sunday were at 12, Monday back down to 7.2, Tuesday back up to 12. So tonight it's opened up to 15 to see if I go higher than that. She's noting that there is lack of snores and it appears I'm finally after a year getting lengthy periods of REM. It usually a central gets triggered and I wake up. My sleep hygiene has been really good. In bed by 1030 and lights out by 11 or 1115. And I'm usually out by 12. I was having a period of a few months of randomly waking up at 4-5 am would be awake for an hour and I'd finally go back to sleep and wake up at 730-800. So right now with the amount of flags she sees she has reason enough to maybe call me in for another full night study (my 5th). Are centrals and high epap related in some ways. As I rarely now see my periodic breathing say 1% seen 5% on a bad night but for the most part the ASV handles it and I see 0%. Running humidity at level 3 hose is at 4, I do drain the tank nightly. Room temp is 66-67.

Apologies for the lack of charts as I am using Encore Pro with my Doc and asked I not share my data as sleepyhead is not compatible with the ASV unit.

I am hoping these machines last a long time. In the 262 nights I've been on ASV I've already put 2050 hours on my unit.

Hoping it's something simple. My lungs are killing me and a few times the pressure on my face was high enough of I don't have the mask cinched tight I will get a leak alarm go off. My mask for has been 99.9%. and no signs of major leaks during the night as again the alarm goes off if my leak exceeds 30 for more than 15 seconds.

Hope everyone had a great holiday.

any ahi on asv is obstructive in nature, as the CA are 100% responded too, according to resmed. It is only a problem if there isn't enough max PS and that is found in the tidal volume and flow rate. Any AHI is normally resolved by increasing min epap and opening the max epap. I would actually look to trialling a resmed asv, consensus on a forum is that they can work better
