CPAP not working

Post by AHItoohigh » Mon Aug 01, 2016 3:05 pm

Hello all. New to the group. Hoping you can share some of your knowledge with me.
I have been using a Philips System One for a little over three years. Nasal pillow mask.
The pressure was first set to 12. Wasn't having luck with that so the pressure was dropped to 6. Still ineffective so the pressure was set to auto. While on auto the pressure would be anywhere from 6-15. Still wasn't helping. At that point I was sent for a third sleep study. The pressure was changed again to 8. I have been at an 8 for the last few months. I feel terrible on a daily basis. My AHI is usually at least 11, and is often into the upper 20's all the way up to the upper 30's. Last night my AHI was 51. Doctor keeps saying to stick with it but I'm frustrated to the point im about to just get rid of this machine altogether and take my chances sleeping without it. If I'm routinely getting AHI in the 20's and 30's is there even any point to me using it?

Thank you!

Which SystemOne auto machine..
What is the AHI made up of? The event category breakdown.

To offer anything other than a wild ass guess as to what is going on we need more information.
Exactly which machine, exactly what settings...plus we need to see a typical daily detailed report from the software that is available.

Software information available here
https://sleep.tnet.com/equipment

How to post images of the reports so we can see what you see
https://sleep.tnet.com/reference/tips/imgur

How to organize the graphs if you choose to use SleepyHead...there's 3 pages...read all 3 please
https://sleep.tnet.com/resources/sleepyhead/shorganize

For examples of what is shown on the reports see this thread
viewtopic/t103468/Need-help-with-screen-shots.html

It's possible that your pressures aren't optimal. It's possible that leaks are a problem.. It's possible that the type of apnea events you are having are made worse by cpap pressure...lots of possibles and we don't have a clue without further information.

Thanks for the reply Pugsy. Tried to give you all the info I could think of but figured it may not be enough. I will check out the software and hopefully report back to you soon. Thank you very much!!

Sounds like the doctor was making random changes without actually looking at the data.

once you get the software going, post some screen shots of the graphs. Then we can help you zero in in better settings.

Who would have thought it would be this challenging to sleep and breathe at the same time?

You need to post your sleepyhead graphs here for others to help.

The break between June 16-July 27 is when my machine was being repaired. Thanks in advance for any advice or tips!

Next time omit the snore graph. It's not needed. Snores are shown on the Events graph up at the top if you didn't squish it so much. The Events graph much more important so...lose the snore graph and make the other graphs a little bigger.
No need to redo this one though.

Your pressure isn't sufficient.. 9 cm fixed isn't enough to deal with the OAs and hyponeas.
The ClearAirway/centrals....I am hoping those are related to OA and hyponea arousals and will reduce if the OAs and hyponeas reduce. If they don't and they get worse with more pressure we get to have a different discussion

Would you mind please choosing the linked BBC code AFTER you choose large thumbnail so that the image will show up in the body of your post? As it is with the links it takes forever for them to load this way.
My internet sucks.

I don't know how much more pressure you are going to need. You have 2 choices...keep increasing the pressure in the current cpap mode until you find an optimal pressure or change the machine over to auto trial mode and set the minimum to 10 cm and max to 20 and let the machine help us find an optimal pressure. The 460 model machine you have should have 30 days of auto (apap) trial mode available.
Leaks aren't an issue. If you have any large leaks it will show up on the Events graph and that graph is so tiny I can't really see the LL line but the leak graph doesn't show you going into large leak territory...so I don't think there will be any LL flags on that events graph and if you did have one it wouldn't be very prolonged.

Your AHI is high because the 9 cm pressure isn't sufficient to prevent the airway from collapsing.
