How do I put my System One into auto-adjust mode?
Brand newbie, just brought home my Respironics System One (560P I think...) last night. It's supposed to be in auto adjustment mode so that the tech can look at the numbers (the modem is attached and working) and determine next steps. At the clinic, I saw that the pressure was changing and adjusting on the fly. When I tried to use it last night, the pressure was fixed at 5.0. Do I need to go to a provider menu to switch it to auto mode, or am I missing something simple?

you probably have the Ramp set to on and set at 5 cm. If so it will not go into the auto adjust mode till the end of the ramp time.
You should get the clinical manual for your machine go here and requesst one :

If you're not part of the solution you're just scumming up the bottom of the beaker!

Re: How do I put my System One into auto-adjust mode?
Thanks! If I'm reading the display correctly, the ramp mode isn't on unless the ramp icon is visible on the display, correct? This happens when you press the ramp button. If that's the case, ramp is not on. I have tried it, thinking it might go into auto adjust at the end of a ramp cycle, but it did not. I'll request a clinical manual, thank you!

karma, there is a short video tutorial -- link below in my signature box -- that will show you all the menus and settings including the clinical menu. You want the last video on that page.

Re: How do I put my System One into auto-adjust mode?
Thank you kaiasgram! I was able to get into the provider menu and see that the unit is in Auto mode.
Is there anything else I can check? Is there any other reason that the unit would apparently stay locked at 5.0, when I saw it adjusting at the clinic, as soon as I put the mask on?

Re: How do I put my System One into auto-adjust mode?
If it already is in "auto" mode what are the settings for minimum and maximum pressure?
It's possible that the minimum pressure is set to 5 cm (4 is as low as it will go) and in auto mode it will stay at 5 cm until some sort of apnea event drives the pressure up and normally you have to be asleep for that to happen. We can't normally cause the airway to collapse while awake which is what drives the pressure up. If we try to hold our breath to fool the machine into increasing the pressure the machine won't do it because if we just hold our breath the airway is already open and the machine will know that and it won't/can't respond if it thinks the airway is open.
It is very possible that the pressure is increasing while you are sleep and you just don't notice it. Use the software available (like SleepyHead) to see what the pressure is doing.
It is also possible for someone to have set the minimum and maximum pressure to 5 cm and if that is the case then the machine can't increase the pressure beyond the maximum setting and with minimum and maximum both set to 5 cm the machine can't do anything in terms of increasing the pressure.

That's good thinking Pugsy, but the levels are set at 5min, 20max. Therefore the most likely issue is me, not the machine. I'll try it for 20 or 30 minutes. It felt to me as though not enough air was flowing in, which made it pretty uncomfortable to sleep.

Many people find this to be the case .... if so you might try a higher min pressure.

Re: How do I put my System One into auto-adjust mode?
That stifling feeling with those lower pressures is quite common. While technically we won't/can't suffocate it sure doesn't feel that way and it is uncomfortable. Even my sister complained when I was screening her for OSA and said I was trying to kill her by suffocation at 5 cm. We ended up having to use 6 cm just for comfort.
If you are uncomfortable and can't go to sleep at the starting pressure of 5 cm...just go into the setup menu and change it to something more comfortable like 6 or 7 cm. There's a good chance that you would probably need more minimum anyway if your pressure needs get up to 10 or more. Gotta get to sleep first so use whatever is more comfortable for you to start with so you can get some sleep.

if your leaks are too large, the pressure won't ever increase....so that's something to be aware of.karmamatic wrote:Brand newbie, just brought home my Respironics System One (560P I think...) last night. You can test the auto-adjustment by making snoring sounds while watching the screen. I tested my DS560 a few minutes ago. It stayed at my starting pressure of 14.0 for a moment, and then it quickly shot up to 15.0 and stayed there. A few seconds later, I was no longer able to make snoring sounds.

I think it was a combination of things, one being not really knowing what a good seal on the mask feels like, and the bottom pressure being a bit too low. It seems like 6 or 7 will be more comfortable, and I practiced holding the mask over my face for a bit, and forcing a snore, which did make the auto adjustment kick in. Thanks to everyone who replied, I'm benefiting from your experience.big_dave wrote:You can test the auto-adjustment by making snoring sounds while watching the screen.
