Well, maybe Pes was the "Gold Standard" (an over-used term to attempt to achieve the intellectual upper-hand in debate) in 1991:
The gold standard for measuring increased respiratory effort is esophageal manometry (Pes), a catheter used to observe the changes in chest pressure to detect periods of increased respiratory effort that lead to disruptions in sleep continuity (5).

5. Shepard JW Jr, Gefter WB, Guilleminault C, Hoffman EA, Hoffstein V, Hudgel DW, Suratt PM, White DP. "Evaluation of the upper airway in patients with obstructive sleep apnea." Sleep 1991,14(4):361-371.
but it sure ain't today.

Excellent point.

During my November sleep study, a pressure transducer was used to measure RERAS which I understand is totally appropriate. While I wasn't officially diagnosed with UARS, I have an RDI of 23 with an AHI of 5.9.

Current sleep doctors called my condition very mild. Yeah right. Unfortunately, she is typical of most sleep doctors in my area who think UARS is a foreign language.

Anyway, after chickening out last year, I am strongly leaning towards having a septoplasty/turbinate reduction to improve my ability to use the pap machine. Just need to find the right doctor but I will.
