What is a good AHI with ASV? Revised

Post by EMTunit31 » Thu Jan 01, 2015 12:52 pm

!st night on ASV and my AHI was 20.34. Shouldn't it be much lower? I realize it started at 107 but I now have the Cadillac of machines (Thats what the tech said). I took right to it like I hoped I would. Slept for 13 hours. I was tired.

Re: ASV AHI number still too high?

Post by Uncle Flapp » Thu Jan 01, 2015 1:14 pm

What machine are you using? What is the breakdown of your numbers? It is common for my ASV to report a high number of hypopneas but CAs and OAs are very low.

That is a high treated AHI, but without knowing more details on the types, frequency timing of events, leak rate and your settings, there isn't much to suggest. Give it a week and see if things settle down. You should download Sleepyhead software to view the graphs of your sleep events and determine the type and duration of apneas.

Most days I am around .5 so yes it should be lower. Please list what your settings are so others can help you. You can put your machine info in your profile so it shows up when you post by going to the user control panel at the left top and choose the "profile" tab. Then on the left you will see, "edit equipment". If you choose text it will show up in words instead of pictures.

An OA index of 0.14 and a CA index of 1.01 are actually pretty good but with a leak rate above the acceptable threshold 28% of the time, I would focus on getting a better mask fit before worrying too much about the numbers. There may be additional events during that time that go undetected (and untreated).

Nearly all of your AHI consists of Hypopnea events. That is a lot better than the breathing cessation types of apneas. It sounds like you got a great night of sleep and felt better, so that is a good sign as well. Hypopneas frequently occur at the lower threshold of effective treatment pressure, so it may be that you will eventually need to raise your minimum EPAP above the current level of 4 cmh2o. The machine automatically titrated an EPAP of 13-14. Studying the graphs might give you some insight on where the hypopneas are occurring in terms of your sleep cycle and pressure.

I think the best advise at this moment is continue with the current therapy and consult with your doctor if these hypopneas continue to be prevalent. It is likely these events might be responsive to changes in IPAP/EPAP pressure settings, but it's a little premature to mess around with it on the basis of one night.

there's *nothing* wrong with the leak rate reported there. nothing at all.

respironics machines report *total* leak, not excess leak. if it was a resmed machine, your comment would be valid.

the statistics page doesn't give much useful information, it would be better if you gave us a screenshot of the daily page, use the f12 or view/take screenshot, then post that.

on the right side, get the event flags, flow, pressure, leak, and snore graphs please.

based on what little there is on the stats page, it looks like you need more epap pressure to cut down on the hypos.

why you no use f12/take screenshot?

it automatically gets rid of the useless calendar and right sidebar that just obscure useful data.
