mask selection / questions after first night

Post by pranayama » Fri Sep 05, 2014 1:44 pm

Thanks everyone for your encouragement. My first night went pretty well. I enjoyed breathing the pressured air. Woke up a few times, and by morning, had light headache. Sleep deprived today, but no worse than usual. Feeling hopeful.
My sleep dr gave me a prescription for a specific mask (Resmed mirage Fx nasal mask). This was the one I wore in my sleep study. (I was told during the sleep study, that the sleep study was not for testing different masks.) I wore it last night at home for first time, and it was okay. Wondering if I would be more comfortable in another type of mask. At Apria, I was told I could exchange my mask within 30 days, and I want to act on that soon. Want to try the pillow mask, or Wisp mask. Any thoughts?
(Likely my sleep dr can help me with further choices, such as a trial of different masks, or a new prescription. Not sure how it works with my insurance.)
More about my first night. Tried sleeping on my back, which I stopped doing decades ago due to snoring waking. It was fun to try new sleep position. I took my chin strap off at 2 pm, because my mouth seemed almost glued shut, so thought maybe didn't need it. After the strap was off, I woke up, snoring, from supine position. If I'm understanding correctly, that's because...without the strap, I opened my mouth, disrupting airflow from the CPAP, and my airway constricted in response?
I'm probably asking basic stuff, and should do some more reading. I also don't fully understand leakage. Are those little vents on either side of the mask? So some leakage is necessary? But ideally, mask shouldn't leak through the soft seal where it touches my face? I turned leakage alarm on, but noticed it only activates with extreme leakage (when I took mask off with machine running).

Couple of things to think about:

1. In general, sleeping on your back will make most apneas worse - can you sleep on your side?
2. In general, mouth breathing is quite rare and can often be fixed by nasal rinses and/or corticosteroid sprays (not decongestants)
3. In general, nasal pillows are the least intrusive, easiest to manage leak wise masks - small, light and sales by being stuffed up your nostrils!

I went through six masks before settling on the P10, with which I am very happy. And I thought I was a mouth breather. I got 3 from my DME (two samples) and paid for 3. I figured out nasal rinsing early on and now get 8 hours or so, and an AHI of <2.

Good luck, it'll take a while but you'll get there. And you'll feel so much better.
