Need help with the settings on Airsense 10 Autoset for her

Post by PollyCT » Wed Mar 11, 2015 3:30 pm

Well guess who has her first ever APAP machine? Me!

Thanks to all the help I received on another thread, I'm all kitted out with the new machine and mask. (P10 Pillow mask for her). I've found my way into the settings, and would love some guidance on how to begin. I know my prescription said 9CM, and that's it. I haven't yet received the sleep study report, but hope to tomorrow. Do I need to wait for the study to arrive to try the machine? If not, how would I set the following?

Max Pressure (4-20)
Min Pressure (4-20)
Ramp Time (Auto – 45 mins)
Start Pressure (From 4.0)
EPR (On or Off)
EPR Type (Ramp Only or Full Time)
EPR Level ( 1 – 3)
Climate Control (Auto or Manual)

I'm heading out to get some distilled water. Hoping to get to try this puppy out tonight, and start on the road to getting a restful night's sleep!

if I'm understanding right, and your rx was 9, and you want to set the machine to be in auto mode, I'd suggest something like
min 8.6
max 11
ramp auto
start 6
epr off
climate auto

that gets you starting close to the suggested pressure, and gives a little leeway in case the machine thinks you need more.

little ramp to get you started, but not at the minimum pressure that many people feel is almost suffocating (it's not, but some folks feel that way)

check your data in sleepyhead tomorrow

good luck!

also, I suggest using the machine for a while before bed, just to get used to breathing with it, read a book, watch tv for an hour or two... something to get you used to the feel, without sitting there thinking about it... distract your mind

again, good luck!

I'm removing this post -- the full spectrum of EPR options are better described below and I misunderstood them on the machine...

Ramp is a feature to allow the user to gently get to sleep before the AS10 enters the therapy mode starts along with event recording. The EPR can be set in the setup menu to allow the patient to adjust it. The ramp only setting means that it will only be used during the ramp mode and not the therapy mode. There is/was a "Full time" setting for the EPR to me used all the time. The values are: Off, 1, 2, and 3. The number values are the pressure reduction in centimeters to the normal pressure. Example: you have the AS10 set to 10cmH2O, a setting of "1" would make your exhale pressure a 9cmH2O, a setting of 3 would yield a exhale pressure of 7cmH2O. Also, the EPR lowest pressure value is 4cmH2O. This means that if you start with a pressure setting of 6 and your EPR is set to 3, it will only drop to 4cmH2O (an EPR of 2)

PaleRider gave you a good starting setup. Below a setting of 6 makes most people feel like they are suffocating. You ought to play with the AS10 before going to bed so it won't be a shock to you and your system.

Thanks, palerider, especially for the last part. I'll do just that! I'll post results tomorrow - I'm sure I'm going to need help understanding that, as well!

Cheers,

Polly

Pale Rider has already given you some great guidance, and I don't want to complicate any issues, but I notice you indicate that you have the "For Her Model" .....if I was a girly girl or even if I was a girly man I'd sure be curious to try out the "For Her" mode and see how that works out for you.......it's supposed to give you a little gentler pressure response to events and be more in tune with female respiration differences. Also will give you RERA data which the regular AutoSet does not do.

I figured that part was just a given

use the 'for her' mode.

Yeah probably is a given.......I am a master at pointing out the obvious.

TangledHose and palerider - definitely do not assume I know anything at all, but in this case I had actually assumed the "for her" settings. Still not really understanding how it can be different, and thinking it's sort of a marketing gimmick, but I liked the white model with the leaves on it better anyway.

I think ChunkFrog has been running hers in the For Her mode, maybe we can get some info from her about how she likes it, I think she had the S9 before.

Good luck and let us know how it goes

when 'for her'ing:
  • reduced rate of pressure increments designed to help prevent arousals
  • slower pressure decays
  • Treats apneas up to 12 cm H 2 O and continues to respond to flow limitation and snore up to
    20 cm H 2 O.
  • Minimum pressure (Min. Pressure) that adjusts according to the frequency of apneas:
If two apneas occur within a minute, the pressure reached in response to the second apnea will
become the new minimum treatment pressure until the next treatment session.

If your prescription says 9 cm., why wouldn't you at least TRY that pressure (to either validate or disprove the effectiveness) before experimenting with ranges?

So... not having seen my actual sleep study I don't know HOW good this is, but this seems pretty good, right?

Because I have more faith in the technology of the machine than I do in my doctor.
