Sounds like your DME is a Fee for Service. Where I used to work we had to bill a certain ICD9 code for CPAP setups. Another ICD9 code for Mask fittings. And another ICD9 code for service machine. Yet another ICD9 code for a RT visit. They are all allowed and billable by medicare.

I believe the charge is high because the law requires a registered respiratory therapist to set up with prescribed settings, explain use, & how to clean. $300+ seems high.
