Here is my asv tritration. Still not feeling any better. I have an appointment next week. Any thoughts or comments welcome.
The patient underwent asv tritration with complete polysomnography and total recording time of 423 minutes with a total sleep time of 274 minutes. Sleep onset occurred at 33 minutes. Sleep architecture revealed 8% stage N1 sleep, 66% N2, 9% N3, and 16% rem sleep.
ASV was applied and tritrated to an EEP of 7 cm with pressure support of 4 to 15 . The patient was asked to sleep supine but was unable to go back to sleep therefore no supine sleep was recorded. There was good control of central and obstructive apneic events on the final setting with a residual AHI of 0. On the lower settings of ASV there were a large number of central apneic events. These corrected with the tritration to an EEP of 7 cm. Mild bradycardia was also noted, with average heart rate of approx. 44.7 during the night. There were periodic limb movements but arousal index was only 2.8 events per hour.
This study is a fair tritration of ASV for control of complex sleep apnea. There was a lack of supine sleep and reduced sleep efficiency during the night. There were minimal periodic limb movements.
Home trial of ASV with EEP 7 cm, pressure support 3-15, and clinical followup to assess response to therapy and treatment compliance.

My question is. On the lower settings of ASV there were a large number of central events. What treats centrals? Epap or IPAP?

epap keeps your airway open.
