so I guess when a PAP machine is sold with a bacterial filter, it is placed so as to filter the incoming air, i.e. right after the usual filter?

I've not seen those; but I thought it was at the end of the tube, an extra-cautious measure to prevent possible bacteria growth in moist areas, i.e. especially the hose, to reach the patient. The usual air filter alone will not block bacteria from entering the system, where the path is dry until you get to the humidifier.
In hospitals, where is it placed?
