An active sleeper tried many mask, failed to solve leakage.

Post by APNEA-C » Fri Oct 26, 2012 4:06 am

I am new to CPAP(ResMed S9 from CPAP.COM).

My problem is I am very active from side to side when sleeping. I tried these masks below and failed to solve the problem of leakage:

ResMed Mirage Activa™ LT Nasal CPAP Mask with Headgear - Large
ResMed Mirage Swift™ II Nasal Pillow CPAP Mask with Headgear
Circadiance SleepWeaver elan™ Soft Cloth Nasal CPAP Mask - Starter Kit
ComfortGel Original Nasal CPAP Mask with Headgear

Every time I turned around, the cushion or headgear contacted the pillow, then the leakage occurred.

It's very wastful and suffered to buy and try so many masks. How can I do now? Can anybody give me some advice?

Most masks will work okay with side sleeping and pillows *IF* they are fitted properly. The main problem is that it is difficult to fit a mask, especially for a beginner. Some of these masks may not fit you no matter what you do, or they may fit you well, if adjusted a little differently.

Please telll us about each mask, what problems it gave you and anything you tried. Then people who have used these masks can help you make adjustments.

For example, I have the Quattro FX (sorry, I don't have any of the masks on your list). I knew it fit well at the sleep study, but I was having a horrible time with leaks across my eye. Once I said that on this forum, several people told me that it was too high on my face. Try using it a little lower. And that solved my problem. I didn't realize I was using it too high.

With just about every mask issue, somebody here has been there, done that, and can offer you suggestions. They just need to see the right key words such as where it is leaking, what it is doing.

Who would have thought it would be this challenging to sleep and breathe at the same time?

Thank you. I will continue to try and report the result later.

Here were problems when using these masks in last 2 months:
1.ResMed Mirage Activa™ LT Nasal CPAP Mask with Headgear
Forehead Pad caused pressure erythema on forehead. Upward air leakage when side sleeping.

2.ResMed Mirage Swift™ II Nasal Pillow CPAP Mask with Headgear
Air leakage when turning side to side.

3.Circadiance SleepWeaver elan™ Soft Cloth Nasal CPAP Mask - Starter Kit
Frequent air leakage when side sleeping. Difficult to fit well. Hose caused cushion to become deformed when turning over(then leakage occurred).

4.ComfortGel Original Nasal CPAP Mask with Headgear
Severe pressure erythema on nose.

ApneaC, I am still a tumultuous sleeper after 16 months.
I'll dare to say that whether you're the patient or the bed partner of patient, the noise and bother of leaks is about the worst hurdle for the first couple months. The Mrs uses a FFM and it was torture while she was oblivious, leaking loudly. I had to move to the couch to get any rest. Then I started, with Sleepweaver. That was a huge challenge, as the ones over a year ago were pretty leaky. EVENTUALLY, one learns to make a small tug while half asleep to correct the leak.
BTW, I tried the eLan and found it too leaky as well; I am now on my third SleepWeaver and won't use anything else. As others have said, it taked persistence and patience to crack this therapy. Good Luck!!

How about routing the hose upwards over your head with enough "play" in it so that when you toss and turn the hose is up and away and less likely to pull on the mask?

There are any number of hose routing options and you can even make your own.

https://www.cpap.com/productpage/HoseBu ... ystem.html

or this one which would be easy to make
http://www.pur-sleep.com/products/prodD ... rch=&page=

I use the Swift FX nasal pillows. I toss and turn and go from one side to another all night log. I haven't had the need to route the hose over my head but many people do prefer to do it that way.

Also...maybe a different bed pillow???

You did not mention hose management. My hoses were all in danger on the bed.
I hang my hose from a wall bracket, with stretchy hair scrunchies, just high enough to prevent tangling when I sleep.
I devised an elastic hanger for the elbow, (2 sili-bands, a little plastic ring, and dental floss)
and found it to work better maintaining mask seal than the hose tether supplied.

I also think hose management can help you a lot. I have a very small hook in my ceiling above my neck and use a simple elastic stretch cord to hold my hose, like this:

This works very well, I can turn completely free from side to side and don't even notice the hose anymore. There is never any pulling on the hose that can cause leaks.

Comment,

The Activa and the ComfortGel are well supported by the headgear and their cushions should not got dislodged so easily.

My guess is that the leak occurs from your open mouth, in which case a chinstrap could help.
I would wear the chinstrap's straps under the mask's headgear straps.

If you post a Leak graph, it could show if it is a mouth leak.

p.s. you could make an experiment by tying your torso to the bed with a strap, the strap going under the bed and back up on the other side (like tying a mattress on top of a sedan). By not posting your gender I assume (at 95% confidence level) that you are a female, so you would place the strap below your breast.
Let's say that you spend half the night like that and then check the leak for that time. If the leak remains, it sould indicate a mouth leak.

Thank you all for sharing experience.

I will continue to wear SleepWeaver elan and advanced, at least they are comfortable and light.

I had a better sleep with fewer leakage events last night with SleepWeaver advanced.

The important tip is hose management, arrange the hose as the instruction DVD's demo.
