The APAP machines won't try to blow through an obstructive event... it will wait and think about what to do and evaluate any event precursors and decide on appropriate action. They don't act like ventilators.
This is one reason why people who have a lot of "frank" apneas (that appear out of the blue with no warning signs) may not have the best of luck with auto adjusting pressures.

Of course if an event is central in nature they don't do anything either.

If there were no other event precursors during that 67 second obstructive event the machine didn't have anything else to evaluate so it didn't know what to do. Might have been a "frank" apnea. No way to know. That might explain the lack of response or it may simple be that the pressure didn't get to where it needed to quickly enough to prevent the apnea and once the apnea has actually occurred the machine won't do anything.

Yeah, it seems it must have been a frank one because there was nothing else around it.

I'm just getting a little concerned because their nurse practitioner was talking as if the number doesn't go down they might stop CPAP treatment, which I know I can't sleep without it. I can't convince them that my number has got to be worse at home than during the sleep study, because I've seen improvement. I just want to optimize it. But then again, I don't think she knew what I was getting at or my purpose of the visit... She was not at all familiar with my chart and didn't even realize she was the one that had sent me for the second sleep study and the daytime one. So who knows... I really think that apnea is the problem, though, and it's not just all "in my head" and a distorted perception. I've pretty much stopped having the drowning nightmares now. I still wake up at like 3 AM and 5 AM like clockwork and some nights are totally settled, so I think that could be why I get tired faster during the day and sleepy when I'm sitting still doing something and not staying active.

The last two nights I've kept my min. pressure at 8. The Saturday's number was 3.76. I slept fairly sound all through the night. Maybe two awakenings or so. Sunday's was 2.37, but interestingly enough I woke up more. Maybe I am just crazy... I know when I set my pressure to a steady 4 one night it was up at 6.67. I just wanted a "baseline" comparison.
