Drs Park and Gold and best pressures for UARS

I’m feeling confused by conflicting information. I have consulted Dr. Park and Gold, both of whom believe in UARS (I have apneas but more RERAs) and they seem to have different philosophies about how to treat. Dr. Park indicated (and I also read on this board) that UARS may require higher pressures to treat successfully than apneas. Dr. Gold says that he treats UARS patients successfully with nasal masks and pressures usually under 10 or so. This will resolve flow limitations not during REM sleep, and he thinks that is enough – not sure why. I noticed in the teleseminar interview that Dr. Park asked Dr. Gold about how his protocol works out in REM sleep – don’t recall the answer. Does anyone have any thoughts on this?

When I was first diagnosed I was sent home with a ResMed autoset 9 with the factory settings of 4 – 20, which I have read on this board is too wide a range. I’m ready to go be titrated in a sleep lab but want one that titrates for flow limitations, not just apneas and hypopneas. Does anyone know of anyplace in Manhattan that does that. Is there a sleep lab that does better with UARS than the others?
