CPAP pressure vs Flow Limitation

Post by Mr. Cellophane » Mon Dec 19, 2011 1:47 pm

I recently made some changes to my therapy which yielded an average AHI of <1.o with minimal flow limitation at a cpap pressure of 7cm. Thinking maybe I could possibly get off the hose completely, I decided to see what happened to my AHI at a pressure of 4cm, which I set for last night. Although I had a restless night and only slept about five hours , I was quite encouraged when I checked my AHI this morning and found it to be 1.0. I downloaded the info into ResScan and took a look. WTF!!!! My flow limitation graph was ridiculous. I googled "apnea flow limitation" and found an article which contained the following:

"At high CPAP, resistance was low and inspiratory flow contour was found to be rounded. At low CPAP, resistance was high and flow contour developed a plateau suggesting flow limitation. We also noted that the CPAP pressure at which high resistance developed, and at which flow limitation appeared, showed hysteresis. We conclude that when respiration is stable, the contour of inspiratory flow tracing from a CPAP system can be used to infer the presence of elevated upper-airway resistance and flow limitation."

What I glean from the article and my graphs for last night is that even though my AHI remained low, due to the low cpap pressure (4cm) I started to encounter Upper Airway Resistance Syndrome (UARS) which disrupted my sleep enormously even though I was not experiencing apnea events as such.

I am posting last night's graphs and hope to receive comments from the experts. Thanks.

FWIW, For last night, I increased my cpap pressure to 6cm (from 4cm). I slept much better, my AHI remained at 1.0 and 90% of the flow limitation disappeared. Plan to increase back to 7cm tonight.

Reply,

In my case I am at a max pressure of 12 cm and a min of 9 cm but my Flow Limitation is still Hi.

See it here:

The Flow Limitation originates at the bottom of the tung indside the throat. If I raise the pressure then new Central events would be emerging. Your low pressure of 7 cm reminds me of my as a result of my PSG at the Sleep Clinic from a year ago. But I kept raising it to get rid of obstructive events, and here I am. I plan to repeat the PSG Sleep Study in two weeks.
