This is great information!  
Note that the "process" is long with an oral appliance.  You have to slowly adjust the device to avoid those jaw pains (and other potential problems).  I kept a journal for about two months where I recorded the number of turns on the device (each turn resulted in less than a mm in moving my lower jaw forward) and the resulting AHI's.  At the end, my oral device is almost maxed out.  But it works great.

Thanks Mike. I didn't realize the adjustment was done so gradually. Reminds me of the braces I had when I was a kid.
They made the moulds this morning. It's a "Thornton Adjustable Positioner" (TAP) first generation, which has gotten good reviews.
Hopefully I will get the same sort of results you did. I'll keep everyone posted.
