Now I am hitting 30cmH2O/taking mask off.

Twice in the last month I have woke up hearing Hurricane Katrina next to my ear. It is my mask. Once I realize what is happening, I turn the SV off, pull out the card and go read it. Yes, even at 3AM I go read it.

Last night I was at 30cmH2O for about 15 minutes, SV dropped my rate of breathing from 18 to 8 and Patient Triggered Breaths went to approx. zero. That is not unusual for me. What was unusual was that I pulled the mask off for reasons unknown. I checked for large leaks and there were NONE. So I must have woke up right after I pulled the mask off??

We have raised the titration on my oxygen (50ft green hose) to 3LPM to see if that might help.

Again, have any of you woken up with your mask in your hand, only to find you had extremely high pressures just prior to that?

I find it incredible that I appear to be the only person on this forum to have this problem. For those of you who have a machine that will reach 30cmH2O, do you have it set there? I am not trying to be nosy, I am hoping to learn from others on the forum. Before it is too late....

Sorry to hear of your situation! Unfortunately I cant totally respond as Ive never awoken with mask in hand. Im on the Resmed S9 Adapt ASV, which has a max pressure of 25, so thats the highest I ramp up to. Apparently, 30 is what you need to keep breathing. I often times am at 23.9. Last nite I has a period of 10 minutes with periodic breathing, one central event after another , that scored on REsScan as just 1 long event! The thing is the Adapt is so smooth, im really never aware that the machine is fighting for me all nite long. Occasionally, I will feel the pressure bumps as Im just drifting off to sleep. Often i sleep all nite long, never waking up, until its time! Maybe, you need to talk to someone and adjust a setting. The other thing is your waking up to loud mask noise! You need to concentrate on getting your mask leaking to near zero. Is your machine ramping up to max pressure to treat your breathing disorder or to compensate for excessive mask leaking. A ASV machine can ramp up to 30 for either one, or combination of both. Best wishes, Dan

Hi Dan,
My leakage is within the tolerances set by Resmed. That being said, they did not have any official leakage figures for 30cmH2O. I called them looking for that info. I have always had a certain amount of leakage, quite normal per Resmed. In the 20s if I remember correctly.

I have slept through 30cmH2O for over an hour recently, no ill effects. BUT, the last two times I hit the 30 number I woke up with the mask in my hand.

I received my new mask and it may have solved the mystery. That was why I had not answered you sooner, I wanted to see what a new mask would do. Last night I had numbers in the mid 20s and slept through it. Might be that the old mask was worn out. It was almost a year old. Pillows were about a month old.

Yup. On my "bad" nights, when I know I will have problems with patient triggered breathing ... the machine does more for me, it's not altogether unusual to awaken with the mask on the bed next to me. It does not happen often, but I suspect (though have not checked the numbers) that the pressure was ramped all the way up. On those nights it starts out high and stays there. I tend to start the night with a REALLY long apnea (more than a minute at a time).

However, this does not seem to be a common / on-going problem. So, usually after a day or two I sleep through the night without taking off the mask.

John, I find it interesting that you know ahead of time when you will have problems with "Patient Triggered Breathing". I never know before hand. My machine starts out low and if it is needed, sometime during the night it cranks it up. I have to wear a nasal device, as opposed to a full face mask because of other lung damage that causes the lungs to expel sputum 24/7. Awkward at best.

I had one apnea last night that was for 15 consecutive minutes. Patient Triggered Breaths dropped into the 50% range and pressure went into the mid 20s.

If you look on the other thread, where I did not breathe on my own for over an hour, no apnea listed. Technically I guess the machine was breathing for me, so I wasn't in apnea.

No sooner do I speak than I started off tonight with 8-10 minutes of solid Apnea. Breathing rate dropped from around 19 to 8 (SV regulating the breathing rate), the Patient Triggered Breaths (PTB) dropped from a solid 100% to around 70%, pressure went into the low 20s until my brain figured it out and took over. Go figure!! I woke up a few minutes ago (after sleeping 1+39)....... coughing. 7.5mg of a strong medicine stopped that. I can't take OTC cough remedies and nothing else seems to stop the cough. Back to bed.
