High AHI

Post by Little_Peep » Fri Jan 07, 2011 7:28 am

I finally got a S8 card reader and downloaded my recent data. My AHI is usually around 10 with a 2 to 3 AI. I notice a cluster of obstructive apneas around 4 a.m., which I think is when I am in REM sleep (how can I tell if that is true?). I also notice that I have a LOT of HI's throughout the night. Does this mean that I need to set my pressure higher to obliterate the HI's or am I just a shallow breather and they are of no consequence?
? I have a titrated pressure of 12 and my S8 autoset is set in the range 11 to 16.

Do you know what your average pressure or your 90% pressure is? If those are higher on your range, you may need to increase your pressure settings. Just be careful not to induce more central apneas. (Does the S8 give that information? I'm not sure) From what I understand, HIs can be just as disruptive as AIs, so you would ideally like to get those numbers down. But, again, the most important question is "how do you feel?" If you are feeling great, then you may not need to mess with anything.

My average pressure is around 11.2. I still feel pretty rotten in the mornings. I can sleep 14 hours at a time and still feel like I have had no sleep. Guess I need to adjust the apap, eh?

A 2-3 AI is quite good. If it's an S8 you have they overcount HIs. Some have suggested cutting the HIs by half to get an HI reading that is closer to that on other models of your machine and on other brands if machines.

Raising your minimum pressure may help with HIs but they will always be higher if that is the machine you have. If you raise it, do it slowly and leave it for several days to let everything adjust.

I would try bumping up your lower number up to 12, leave it there for a few days at least, and then see how you are doing. If you are not spending a lot of time at higher numbers, then I don't think you would need to bump up your upper number. Some people have the best luck with a very narrow range of pressures, so that shouldn't be an issue.

Will do. If i didn't feel so lousy, I would ignore the HI's.

You shouldn't ignore the HIs totally but the S8 does count very high on these.

You could benfit from a gradual raising of the mimimum pressure in the hopes that you may feel more rested and eliminate some of the HIs and maybe lower the AIs. While your AIs are pretty good, you may get fewer of them as well.

How long should I leave it at a pressure before I increase it again?

I would suggest not raising it for 3 or 4 days to a week after a small increase, unless you feel lousy. Then you should raise it sooner, like in a couple of days.

Thanks! I look forward to the day when I awaken refreshed and energetic. It feels like I have just closed my eyes when it is time to get up, no matter how many hours I sleep!

Great news! I have slowly been increasing my lower pressure by 0.2 increments and now have an AHI of 6, with and AI of less than 1! And I actually feel like I have slept! Thanks for all of the good advice!

Wow! My AI last night was 0! And my HI was 4.8! It's working! It's working!

I have a device called a Zeo that records your sleep patterns. I can thus see when during the night I am in REM, deep, light sleep or am awake. I can then correlate that with my apneas. http://www.zeo.com
