ResMed Mirage Quattro - Like No Treatment at All?
I purchased a new ResMed Mirage Quattro full-face mask in March. After using it a month I noticed that all of my pre CPAP treatment symptoms had returned...extreme sleepiness during the day, heart arrhythmia, etc. It was horrible. I'm not a newbie, I've been in treatment for 5 years. I noticed that the new mask seemed to release more air through the exhalation port then my old mask (Fisher Paykel HC407), so I thought that maybe it wasn't keeping my required pressure. The mask fit fine with no leaks, so I know that was not the problem. I decided to go back to using my old mask...and all the old symptoms DISAPPEARED, and I am fine again.
I called CPAP.com and they issued a return to check out the mask. I thought I would post this here in case someone else is experiencing the same trouble! I will post the results of what CPAP.com might find out.

I've read many instances here of problems when switching to full face, including my own - sleepiness, achiness and generally not feeling well. But I just can't breathe through my nose only.

When I wore my Quattro (it's been a couple of years since I last wore it) I also noticed that "less-than-rested" feeling. However, I believe that it's because of the LACK of exhausting enough CO2. In other words, there's not enough Vent Flow Rate with this mask. There was a thread a year or so ago where "DSM" and I discussed this because he also noticed the same thing.

Thanks Den for that info. But I don't think it's a Pco2 problem..otherwise I would have recovered during that day (at least by noon) and be fine until the next morning again. The whole thing was a complete (and scary) reversal...back to the days before I was diagnosed, I'm talking constant heart palpitations, and inability to focus, feeling like I had been up all night. That to me would indicate that I was having increased apnea episodes with that new mask, showing a lack of enough pressure for proper treatment.
My old mask is also a full face mask, so it has nothing to due with the type of mask. I hope that CPAP.com will find an answer.

Were your therapy numbers (AHI, etc.) any different with the Quattro vs your old mask?

pwpyle, can we back up for a minute? How do you know your Quattro is not leaking while you're sleeping? Do you have a data capable cpap machine which clearly shows you this? This in addition to the question above about tracking your AHI numbers against the leak rate. Not meaning to challenge you, just curious about the details of this phenomenon.
DeVilbiss IntelliPap Std Plus with Smartflex; Transcend miniCPAP & Everest2 w/humidifier & batt for travel. UltraMirage FFM; PadACheeks; PaPillow. Using straight CPAP at 13.0/passover humidifier. AHI consistently < 1.5. Began CPAP 9/4/08.

Along with leaking in the software data, are you having more AHI / apnea events with the Quattro as shown in the software data? I would also suggest that you video record your sleep with one or both masks so that you can "see" what you may or may not be doing to cause events to change. Otherwise, without software data and a visual analysis of several nights sleep, are you not just guessing at cause and effect? Your results may be changing due to a change in sleeping position, even if it is just slight, to cause apnea events. Of course if you are unconcious enough or "asleep", then you won't know this, which is why I suggest video recording several nights and hopefully compare those to your software data.

I MUST stay off my back to reduce OSA & snoring. I use a small backpack of solid styrofoam to keep me on my side (tennis balls too small), & use DIY customized soft foam pillow to keep my head in a side sleeping position to eliminate most OSA.

I am starting to feel in the same boat. My numbers are the best they've been, max AHI of 1.2 lately. Had a few 0.1-0.3 nights.
I wake up tired and feeling like I am deprived of oxygen... I mean I can't wait to take my mask off to get some fresh air. I wouldn't say as bad as having a night of no CPAP but it's definitely impacting me.

When I first started CPAP, I started with the Nasal mask and realized I was needed a FFM because I am a mouth breather... I used the Quattro for a month or so and had horrible results... so I switched back to the nasal and tape my mouth shut... however I do use the Quattro as a back up if I get a cold... and just recently I decided to try it out one more time and gave up half way through the night lol... I feel like I am sleeping better when I breathe through my nose... with the FFM my mouth gets very dry and my tongue rests at the back of my throat blocking the air anyway... Everyone is different though and some just have better results with the FFM then others... I sometimes wish it would work for me so I didnt have to tape my mouth shut.

I think what they did was to try to make a "more profitable" mask with the Quattro......using fewer parts and simplifying the design. In my opinion they came up short if they were also trying to improve on the UMFF. My first impression was that the seal was softer and slightly more comfortable than the UMFF, but overall, I like my UMFF better. And, the Vent Flow Rate seems to be better.......at least I'm not "dragging" the next day.Max250 wrote:I often wonder what the difference is between the UM FF and the Quattro. My husband has the same in-between measurements as you and had the same problem with the Quattro S and M. The UMFF Medium is the right size for him. The UM Small definitely did not fit.
