presure induced centrals ?

I switch between cpap and apap on occasion to see which gives me the better result.I had been on cpap at 9.6cm with good numbers but decided to try apap with a range 8.6-10.6. The first night I had AHI of 1.6 and AI 0. Last night I awoke after 3 hours with a full bladder and to my dismay the AHI was 0 as was the leak. Since I am prediabetic ( probably due to OSA) I check my blood sugar and this A.M. it was 110mg%. It usually runs around 90mg%.I think I had a classic "fight or flight" response probably from central apnea due to low pco2 and O2 desat. It may also be that I am sensitive to apap pressure changes.Suggestions?

Sorry, I'm not understanding your 1st question. If your AHI and Leak were both 0, then you had no apneas or hypopneas--and also no Centrals. On the S8, Centrals would report as an apnea.

When I used the S8 AutoSet II (for 5 months), I could not tolerate the pressure changes of apap, even with a narrow range, but did very well on cpap. I set the cpap pressure at the 95 percentile pressure, and tweaked from there. With the S9 AutoSet, I am using apap with no problems.

I was not aware that the S8 reported centrals as an apnea. Thanks for the info. I guess I may be sensitive to pressure changes even with a 2cm range.Better stay with cpap.

Sounds like I might need to get an S9 if I want to do apap.Do you feel better wih the S9 apap than you did with the S8 cpap?

I'm not sure how to answer that question. I felt good on S8 cpap, and just accepted that apap didn't work as well for me.

The main reason I switched to the S9 was for the Climate Control, and I am pleased with how well that works. I also liked the data available with the ResScan 3.10 software, especially the Central apneas.

I didn't have any expectation that apap would work for me, but I'm glad it does. I prefer apap, because even my small range of pressure (10-12 cm) allows me the comfort of a lower pressure most of the time, with the option of higher pressure when needed. On S8 cpap, I couldn't go above my pressure of 10.6, because of aerophagia. I may have adjusted to that over time, I don't know.

It's a very individual thing. Some prefer cpap, others prefer apap. I don't know that one is necessarily better than the other.

Which means you had NO apneas (central or obstructive).

While OSA can impact blood sugar, so can many, many other things. Don't just blame high blood sugar on OSA. Diet change? Higher carb intake? Late night snack? If you have a pattern of untreated apnea, then higher blood sugar might be due to OSA. But with an AHI below 5. Not likely.

With an AI at ZERO ? I don't think so. Sure you had a "fight or flight" response. But it's not likely it was due to central apneas. I've used xPAP for about 20 years. And there are times I *STILL* want to rip the mask off my face. I get claustrophobic.

That is actually more likely. It could be the change in pressure is awakening you. If so, see if you do okay with CPAP. If so, then stick with it.
