Where can I find a copy of the FOSQ?

Hello everyone. I'm new to this site and to using cpap software and equipment. I have Encore Pro ver 1.8. I finally got it installed and am working with it, but I don't have a copy of the FOSQ. I've searched this site and googled it -- I found one copy but can't be certain if it's the same one used by Respironics Encore Pro. Does anyone have a link to a site with this survey? Or a copy of the items (and the response scale) that are for the Encore Pro software? Any help would be greatly appreciated.

encore pro has it - go to a patient and put in input - at least in my copy of EP the questions are there to be read

Are you sure you have a "Plus" machine as you say in your profile????

You can only get compliance data with a "Plus" and the software is uselss on that machine.

As for the plus machine, that's what I have now, but I have an M series auto cpap with c-flex on the way. I should have it in a couple days and, based on what I've read, I should be able to get full data from this. I'm jumping the gun a bit, but wanted to get the software up and running before it comes. I'm sure I'll be asking more questions over time if I get confused. Thanks to all for your help.

With your software, you will get full data with that machine! You are all ready with the sofware!
